  • Prepare the assignment based on the textbook of the 2021 academic year prepared and published by NCTB. No need for guide books, notebooks, or notes bought from the market.
  •  In the case of assessment, the student’s self-esteem, creativity, and consistency will be checked. So if a student submits an assignment by copying someone else’s writing, it will be canceled, and they will have to submit the task again.
  • Assignments should be written directly in your hand. It will be useful to understand the subject as well as practice handwriting.
  • Use any paper when writing an assignment. However, name, class, roll, subject, and title/type of work should be written on the cover page.

Evaluation Instructions for Teachers:

The teachers will play a significant role in this web-based assignment system provided by the directorate of Secondary and Higher Secondary Education. They have to follow the below instruction for giving, collecting, evaluating, and storing the assignment. 

  • Following the assignment grid (published on the Directorate of Secondary and Higher Education), the student has to assign three assignments (work) per week.
  • The task of submitting the proposed scheme (assignment) of the prescribed subjects, showing the students’ assessment, and the assessment examination comments then storing it in the institution has to be completed by 31st December.
  • Must ensure the participation of each student.
  • Every educational institution has to keep all the assessment records properly.
  • Assignment (work) includes descriptive questions, short answer questions, Creative Questions report preparation, etc.
  • Students will submit assignments on white paper by handwritten [No Computer typing will be accepted]
  • The parent or student’s guardian/ representative must follow the covid-19 rules provided by the DGHS, collect and submit the assignment from the educational institution once a week. 
  • The teacher will pay special attention to whether the student’s writing reflects their essential thinking, imagination, or creativity.
  • The answers given should be consistent with the required information, theories, concepts, formulas, explanations, etc.
  • Assess each student’s assignment in each subject and mark his / her strengths/weaknesses in the notebook; His comments should be recorded so that the student can clearly understand his strengths and weaknesses.
  • After evaluating the assignment, the teacher will make arrangements to reach the assignment with his opinion and will return it at the end of a specified time and save it in the institution.
  • The teacher will comment on the overall assessment of all the assignments in a subject in a class. E.g., (Excellent, good, average, and requires progress). 

(A) Excellent will be determined based on the below context: 

(1) Accuracy and consistency of content. (2) The written information, theories, and formulas are consistent with the textbook. (3) Notable self-creativity or creativity of writing.

(B) Good Comment will be determined based on the below context: 

(1) Accuracy and consistency of content. (2) The written information, theories, and formulas are consistent with the textbook. (3) A little self and creativity in writing.

(C) Average

 1) Lack of consistency, even if the content is good. (2) The text’s information, theories, and explanations are not consistent with the textbook. (3) A little self and creativity in writing.

(D) Progress is required: 

(1) Lack of accuracy and consistency of content. (2) The information, theories, formulas, and explanations in the text are not consistent with the textbook. (3) Lack of self or creativity in writing

Instructions for Heads of Institutions:

  • All teachers, students, parents must follow the rules of health and ensure health protection issued by the DGHS.
  • All subject teachers, other teachers, staff, and all student assessment activities should be ensured.
  • The Education Officers of the Department of Secondary and Higher Education, Regional, District, and Upazila level will monitor the students’ assignment and evaluation process to verify the effectiveness of the student assessment for the web-based assignment system. So these have to be stored properly.
  • Care must be taken to ensure that no student is exposed to immoral pressures. Local education officials can be contacted if any information is required in this regard.
  • Confirm the participation of Teacher students in the student assessment by the assignment. 
  • The assignment grid (published in the Department of Secondary and Higher Education) should be followed.
  • The Head Teacher can arrange an assignment deposit box if necessary, to avoid social distance/student gathering.
  • Authorities have to publish a routine When to submit class assignments at specific times on specific days and distribute subsequent tasks to maintain social distance.
  • If Students submit an assignment by copying from a notebook, guide book, or someone else’s writing, Teachers will need to cancel it and ask to re-submit that assignment.
  • From November 1, 2020, to December 31, 2020 AD. Until then, students will not be given any other type of examination or homework without the assignment prescribed by MAUSI (Directorate of Secondary and Higher Education)
  • Make sure these instructions reach parents and students.

Dear EarthTalk : Aren’t environmental issues primarily about health? Detractors like to trivialize environmentalists as “tree huggers,” but the bottom line is that pollution makes us sick, right? Wouldn’t people care more if they had a better understanding of that?— Tim Douglas, Stowe, Vt. No doubt many of the ways we harm our environment come back to haunt us in the form of sickness and death. The realization that the pesticide-laced foods we eat, the smokestack-befouled air we breathe and the petrochemical-based products we use negatively affect our quality of life is a big part of the reason so many people have “gone green” in recent years. Just following the news is enough to green anyone. Scientific American reported in 2009 that a joint U.S./Swedish study looking into the effects of household contaminants discovered that children who live in homes with vinyl floors—which can emit hazardous chemicals called phthalates—are twice as likely to develop signs of autism as kids in other homes. Other studies have shown that women exposed to high levels of polybrominated diphenyl ether (PBDE) flame retardants common in cushions, carpet padding and mattresses—97 percent of us have detectable levels of these chemicals in our bloodstreams—are more likely to have trouble getting pregnant and suffer from other fertility issues as a result. Cheaply produced drywall made in China can emit so much sulfur gas that it not only corrodes electrical wiring but also causes breathing problems, bloody noses and headaches for building occupants. The list goes on and on.... But perhaps trumping all of these examples is the potential disastrous health effects of global warming. Carbon dioxide emissions may not be directly responsible for health problems at or near their point of release, but in aggregate they can cause lots of distress. According to the Center for Health and the Global Environment at Harvard Medical School, climate change over the coming decades is likely to increase rates of allergies, asthma, heart disease and cancer, among other illnesses. Also, it is quite likely that, as global temperature rises, diseases that were previously found only in warmer areas of the world may show up increasingly in other, previously cooler areas, where people have not yet developed natural defenses against them. And the loss of rain forest that accompanies increases in temperature means less access to undiscovered medicines and degradation of the environment’s ability to sustain our species. Given the link between environmental problems and human health, more of us are realizing that what may seem like exorbitant up-front costs for environmental clean-up may well pay us dividends in the end when we see our overall health care costs go down and our loved ones living longer, healthier lives. Even your local TV station or newspaper likely carries an occasional story about the health effects of environmental pollution. We don’t have to look very hard to find examples of environmental neglect leading to human suffering. But with newfound public awareness and the commitment of younger generations to a cleaner future, we are moving in a good direction. Solid waste has become a tenacious issue worldwide. It has been increasing exponentially due to urbanization and the increase in the population. Since the twentieth-century technological revolution, there have been significant changes in the composition of solid waste. It poses significant challenges for waste management systems worldwide. Waste management is the process of handling waste right from its creation to its final disposal, including transport, collection, treatment, and monitoring. The present solid waste management (SWM) system is affected by unfavorable institutional, economic, technical, legislative, and operational constraints. Poor waste management is affecting ecosystems and human health, damaging our finite natural resources, impeding human economic progress, and harming people's quality of life. Now, researchers are concerned about the environment's degradation, a decline in quality of life, and risks related to waste management grow as the volume of solid waste. So, they are focusing on sustainable waste management practices which will be crucial for creating a cleaner and healthier environment for future generations. This chapter focuses on the concept of solid waste, its types, management, and its effect on the health of humans and the environment.

Environmental issues are nowadays some of the most topical issues in the world. With the rapid development of cities and the increase of population, urban waste management is facing with new pressures and challenges with major potential impacts on the urban environment. The increasing quantity of solid wastes is a growing environmental problem in developing countries like Bangladesh. Pabna Pourashava is one of the oldest Pourashava in Bangladesh and it was established in 1876. The area of the Pourashava is about 16 sq km and its population is 1,27,319.These population generates a massive quantity of solid waste everyday from various sources. According to the Pourashava, at present approximately 52 metric tons of solid waste generate per day. With the existing resources, the Pourashava is able to collect approximately 31 metric tons and dispose of it every day. The rest is lettered around. This research tried to investigate the present solid waste management practice in Pabna municipal area especially in Ward 01 and 02 intensively. It was revealed from the study that majority people of the study area dumps the produced solid waste in their living surroundings which produce various hazards in human health and environmental problems. Moreover lack of adequate budget for waste management some problems appear like as lack of available transport vehicles for waste management, lack of solid waste treatment plant etc. But this improper waste management can be managed by the regularity of management work, building awareness of the people. The municipality and other related NGOs should work together to solve these issues. The total solid waste management system required following the recommendation which include proper planning, creating awareness, developing infrastructure, providing logistic support and finally involving NGOs & public in this process and by this process sustainable solid waste management can be established which can lead the studied area to a healthy setting.

Following the declaration, UN High Commissioner for Human Rights, Michelle Bachelet, said: “Bold action is now required to ensure this resolution on the right to a healthy environment serves as a springboard to push for transformative economic, social and environmental policies that will protect people and nature.”

We asked leading civil society voices what they think is needed from government, business and civil society to take action and mobilize efforts around ensuring and protecting a clean environment for all. Here’s what they said:

We need to reframe our relationship to nature

A reframing of our relationship with the natural world is needed in order to ensure we can provide everyone with a clean, healthy and sustainable environment. For too long we have artificially separated ourselves from nature. We’ve placed an artificial divide between ourselves and the natural world. Our politics, economic models and modern lifestyles reflect this.

This is not natural, and as we are seeing, not healthy or sustainable for the planet or for humanity. The human species is inextricably interconnected with nature. Throughout history, and in all parts of the world, nature is a common denominator. It is the basis of our civilizations, cultures and ways of life. A return to this way of thinking needs to be the basis of any meaningful action to restore the environment, address the climate crisis, and put nature on the path to recovery.

A social dialogue is needed

Realization of the right to a clean and hazardous free environment requires commitment to social dialogue by tripartite partners in reconciling economic and social interests. Although the laws generally make it mandatory for businesses to remedy environmental impacts caused by their economic activities, weak government regulatory mechanism makes enforcement an onerous exercise. These challenges are even more pronounced in the case of multinational enterprises. An equally weak civil society is disabled from monitoring the level of compliance with environmental obligations and provide meaningful contributions to environmental policy formulation and management.

Therefore, states should strengthen their environmental management policies and regulations; they should build capacities of civil society and government entities; provide additional funding for training and education of social partners; establish a tripartite social dialogue forums on environmental protection, inclusive of civil society.

A game-changer for people and the planet

The Human Rights Council’s recognition that a clean, healthy and sustainable environment is a human right can be a game-changer for people and the planet.

But there is much more needed to make this right a reality for all. States must advance efforts to implement the right working hand-in-hand with civil society, businesses and other stakeholders. They must take urgent environmental action, backed by adequate finance, and support a just transition to a sustainable, human rights-based economy. Businesses should integrate environmental considerations in human rights due diligence processes, fully accounting for how the environmental effects of their activities can also affect human rights.

Those with power and access in fora where environmental policy is being developed, like COP26, should elevate the voices and leadership of affected individuals and communities, who are frequently excluded despite often having the most at stake and the best understanding of effective solutions. They should also promote and accept accountability for the harms caused by environmental damage.

The lives of women and vulnerable communities depend on clean air and clean water for all

An estimated 240 acres of natural habitat is destroyed every hour. Women and girls bear the brunt of this environmental degradation. While ~30% of humanity does not have access to safe drinking water, UNICEF acknowledges the 200 million hours that women and girls spend collecting water daily as a “colossal waste of their valuable time.” Evidence shows women's participation and decision making in management of local forests significantly improves forest conditions and conservation. And, leadership by Indigenous women, who have sustainably stewarded our natural world for generations, is crucial to preserving life without perpetuating the undue burden on already vulnerable communities.

Economic recovery and environmental action can go hand in hand

The connection between human wellbeing and nature is indisputable. Governments, businesses and civil society must collaborate on every level to protect the ‘best’ of what is left on Earth and at sea, and improve areas of food production, energy siting, fishing and infrastructure planning. There is a clear path to funding this transformation by reducing ineffective subsidies and supply chain practices, producing new sources of funding and investing in a manner that pays dividends for nature.

In fact, we can close nearly half the biodiversity funding gap with no new funding, by reducing the flow of capital to harmful behaviors and shifting it toward activities that benefit nature. If done thoughtfully, economic recovery and environmental action can go hand in hand, leading to healthier, more prosperous lives.

This is a clarion call for public engagement and accountability

The pandemic has made clear how shared our environment truly is, and also how unequal. Rich, high emitting countries that have benefited from the forces that created climate change are insulating themselves from those effects, in similar ways to the pandemic. This is a moral issue, but also one of justice. The most urgent changes needed are from these governments – crucially, the G20 countries – to reduce their emissions, which accounts for a staggering 80% of global emissions, and to fully transition to green economies. These countries need to grant, not loan, urgent climate adaptation finance that developing countries have long awaited so as to develop sustainably and build resiliency to the disproportionate impacts of climate change on their populations, many of whom are on the move. They also require parallel loss and damage climate finance to avert and minimize effects, and as compensation for the irreversible damage, forced migration and displacement.

Governments must also ratify and update their laws to uphold the right to a healthy environment to its full extent, including holding businesses, particularly extractive industries compliant. Environmental pollution and climate change have shown to be effective issues in empowering people to action, and climate related litigation has proven effective in Germany and the Netherlands , and in individual rights, where in France, pollution was a factor in determining a migrant’s residency rights . Civil society must seize this opportunity as a clarion call to mobilize public engagement and hold governments to account, where the greatest hope is in the resilient Global South and youth, pointing the way forward.

The right to a healthy environment offers hope to those most impacted

The adoption of the resolution recognizing the right to a healthy environment could offer some hope to many communities around the world already hard-hit by environmental degradation and climate change. To make the enjoyment of the right a reality for those most impacted, governments should recognize the right at the national level and develop strong environmental protection laws and policies to safeguard the rights of at risk populations. This includes requiring businesses to comply with environmental and human rights standards, for example through mandatory climate change due diligence regulations, and ensuring broad participation in environmental decision making by civil society groups and impacted populations – including protecting environmental defenders under threat or attack.

Businesses should comply with existing laws and ensure that their operations, including their global value chains , do not negatively affect the environment or violate the rights of nearby communities, including through rigorous environmental and human rights due diligence. They should stop efforts to silence environmental advocates, for example through baseless nuisance lawsuits—known as strategic lawsuits against public participation , or SLAPPs—and align their business models with international environmental and human rights standards.

What does a prepared community look like?

As communities look at how to prepare for the next emergency, they usually focus on stockpiling emergency supplies, having clear alert networks and ways to communicate with the public, and designating evacuation routes and shelter locations. While all of these are key aspects of emergency planning, one area of preparedness that is often overlooked is community health. Community Health is a term used to describe the state of health and how easy or difficult it is to be healthy where people live, learn, work and play. The health of a community, including ease of access to medical care and community resources available for exercise and encouraging healthy habits, is an important part of emergency planning that can have a positive impact on a community before, during, and after a public health emergency.

Unhealthy communities often have a large number of individuals that are more vulnerable before, during, and after a disaster. Factors that lead to poor health in communities such as high rates of chronic diseases like diabetes and heart disease, limited access to general medical care, and low levels of health education, can cause substantial difficulties for a community recovering from an emergency event. Gaps in medical care can increase significantly after a disaster due to physical damage to health care facilities or from a large increase in the number of people who need medical attention. People who already have poor health are usually more susceptible to disease during a public health emergency and cannot get the normal day-to-day medical care they need.

Help promote health in your community by becoming more engaged in your community. Encourage local community groups and government organizations to consider community health in their emergency preparedness plans. Take action to improve your community’s health now to ensure you are better prepared to remain healthy when an emergency occurs.

Living a healthy lifestyle and maintaining healthy habits is disaster prepare kit in it self. Most people think and often we teach that preparing for an emergency it is essential have “emergency items” such as flashlights and blankets, and water. These items are very important as essential needs to sustain life include, nutrition, shelter and water. In the event of an emergency, having a healthy life style gives you an advantage to sustaining life as no one can truly predict exactly how an emergency event will affect them or their immediate environment. Elders are typically the most affected by disasters due to often their health status. Encouraging things such as eating nutritious meals so that your body has sufficient nutrient stores, and performing physical activities to promote physical health and limit the needs of medications and additional assistance that may be very limited in event of emergency is important. Having healthy habits builds a stronger you, building stronger immune systems and fighting infection that can rise to very high level during emergencies as many situations force persons to live in close quarters such as shelters. Helping yourself also helps others!

Vaccinations provide an important defense against disease and infection for both the young and the old. Additionally, a sound vaccination protocol helps protect the general public from a possible outbreak occurring. The possibility of an epidemic re-occurring is a real and legitimate concern, so continued vaccination of the general public is necessary. The health benefits that a proactive vaccination schedule provides for children and the society in which they live far outweighs the potential risk associated with vaccinations; therefore, childhood vaccinations should be administered to every child on a regular schedule.

I solely love the idea of a community coming together to prepare for and work through disease and infection epidemics, as well as injury or disaster incidences. The people are the heart of the community and working together to achieve the same purpose speaks volumes of the community’s residents. Disease prevention and health promotion are important aspects in maintaining the health of the community. Each individual resident plays a personal role in promoting community health. This is achieved when residents exercise 3-4 times a week, eat a nutritious and balanced diet, get adequate amounts of rest and sleep, and stay up-to-date on vaccinations and immunizations. Promoting and maintaining a healthy community is everybody’s business and the responsibility of every individual living in it.

Consuming healthy meals, vaccinating your children, and devising an exercise plan are all steps that can help in improving the health of the community. Vaccinations are necessary in order to protect the youth and the community as a whole. There are health clinics located in many communities to ensure that every child can be vaccinated or immunized no matter what their income is.

It is interesting to note that everyone in the population has a part to play in community health. The health of a community rests not only on the government and policy makers but also on the community workers. Healthy communities recover well because the roles and responsibilities of the community health worker are at play and well followed. The income,race,education or location of individuals is not a stumbling block to getting proper and good health services and resources.

What Is Community Health and Why Is It Important?

The community you live in is part of who you are. Even if you don’t see your neighbors every day, you recognize that the decisions you make impact those around you. You’re all in it together, and you wouldn’t have it any other way!

Improving your community and helping others is often at the top of your mind. So when the phrase “community health” crossed your radar, you had to know more. What is community health? And how does it affect the lives of those in your area?

Community health is the intersection of healthcare, economics and social interaction. Unfortunately, many people are unaware of the role this type of healthcare plays in their everyday lives. Join us as we explore the impact of community health on your neighborhood—and what you can do to improve it.

What is community health?

Community health is a medical specialty that focuses on the physical and mental well-being of the people in a specific geographic region. This important subsection of public health includes initiatives to help community members maintain and improve their health, prevent the spread of infectious diseases and prepare for natural disasters.

“Working at the community level promotes healthy living, helps prevent chronic diseases and brings the greatest health benefits to the greatest number of people in need,” reports the Centers for Disease Control and Prevention (CDC) . 1

Strong community health requires residents to look beyond themselves and take “collective responsibility,” says Caleb Backe, health and wellness expert at Maple Holistics . “It’s not just about the healthcare system, but focuses on the importance of leading a generally healthy lifestyle in order to protect the community as a whole.”

Community health is inextricably tied to individual wellness. “Good community health equates to healthy people, as a community is the ecosystem or environment in which people live,” says Thomas G. Bognanno, president and CEO of Community Health Charities . “It’s difficult to be healthy personally if your community is unhealthy.”

The far-reaching impact of community health

Community health flips the script on the old adage, “You take care of you; I’ll take care of me.” Instead, public health experts agree that the health of a community can have far-reaching—and sometimes surprising—impacts on individual health and beyond.

“Community health impacts everything—educational achievement, safety and crime, people’s ability to work and be financially healthy, life expectancy, happiness and more,” Bognanno says. “Health impacts every other facet of life, from a child’s ability to learn to an adult’s ability to work, so health is critical for education and financial well-being.”

The effect of health on quality of life can also impact the desire to participate in civic duties like voting, social functions and leisure activities, according to Healthy People 2020 , an initiative of the Office of Disease Prevention and Health Promotion. 2

Communities that are attentive to public health can even reduce inequality among their residents. “[Community health] also helps to reduce health gaps caused by differences in race and ethnicity, location, social status, income and other factors that can affect health,” reports the CDC . 1

The consequences of neglecting community health

A lack of focus on community health can lead to a range of complex problems that aren’t easy to correct. For example, crime and safety issues that result from neglected community health can quickly becoming a self-perpetuating cycle. “Repeated exposure to crime and violence may be linked to an increase in negative health outcomes. Children exposed to violence may show increased signs of aggression starting in upper-elementary school,” reports Healthy People 2020 . 2

Chronic diseases, such as diabetes and heart disease, can also increase if a community’s overall well-being is suffering. “An unhealthy community tends to be obese and struggle more from chronic diseases and other health challenges,” Bognanno says.

Chronic diseases like these not only reduce life expectancy, they have a dramatic effect on the economy. The CDC reports that 90 percent of the nation’s annual healthcare expenses are for people with chronic health conditions. 3

Curbing the spread of infectious disease is also a priority of community health programs. Without them, communities may find themselves battling outbreaks of illnesses that put vulnerable populations like the elderly at higher risk.

“If a community has to recover from an emergency event, such as a natural disaster, reducing the spread of disease becomes a crucial part of the recovery process,” Backe says.

Factors that improve community health

Improving community health is a huge undertaking that involves cooperation between public health workers, local government, volunteers and average citizens alike—and the end products of their work can take a lot of forms. “Communities benefit from walking trails and bike paths, from access to healthy food and playgrounds, from accessible healthcare services, schools and places of employment, as well as affordable housing,” Bognanno says.

Education also plays a large role in maintaining community health. Health fairs and advertising campaigns that expose the dangers of risk factors like tobacco exposure, poor nutrition and physical inactivity can raise awareness about the importance of choosing a healthy lifestyle. The CDC has also focused on promoting nutrition guidelines in schools and increasing the amount of physical education children receive. 4

Individuals can step up to the plate for their community’s health by volunteering at health fairs or blood drives, petitioning local officials to develop more green space and walking trails and maintaining their own health. “The collective responsibilities that individuals have for their communal health can lead to positive interactions within the community as a whole,” Backe says.

Can we have a polluted environment and a healthy society at the same time?

A polluted environment means an unstable and unhealthy society. The effects of pollution are so many, as they range widely. It would be hard to have a healthy society in an area where there is pollution. Pollution causes damage to human and animals, plants, as well as the entire environment(Plattenberg, 2007).

Globally, the types of pollution that are common are air, water, and soil pollution. According to several researches done, it is evident that the impact of pollution may range from a minor discomfort to complex diseases. The people who suffer from mild discomfort because of pollution may tend to ignore the issue. The seriousness of pollution impacts is realized when complex diseases arise such as cancer or physical deformities.

Whether minor or complex effect of pollution, all leads to unhealthy living. Air polluted area would probably result to unhealthy people suffering from lung inefficiency, respiratory problems, asthma attacks, and infected physical body organs.

Water pollution would result to serious complications such as typhoid, Amoebiasis, Ascariasis etc. Soil pollution results to worse effects to human beings such as leukemia, brain damage to young children, kidney damage, and fatigue(Rana, 2006). Nevertheless, it would be hard to maintain health in a polluted environment.

Maintaining environment health is a highly crucial aspect in the society. Citizens find it hard to avoid pollution, but there are measures that can be put, to ensure minimized environmental pollution. Any form of pollution, leading to serious diseases like cancer and body organs failure should be avoided totally.

In efforts of trying to handle the issue of environment pollution, there are vital steps that would be considered first. Close observation of any chemical released to the environment without any control would be crucial. All the offensive trades that might be taking place within cities should be inspected to ensure that they are licensed, and their disposal guaranteed.

Any application of intended development that is likely to cause environment pollution should be assessed closely(Malabika, 2009). It would also be the responsibility of citizens to present complains of any form of pollution, which they may experience. Any form of pollution that may cause harm to human and animals should never be given a chance.

In conclusion, it is evident that I have proved beyond any reasonable doubt that, a polluted environment cannot sustain the people’s healthy status. Although Larry used to take water direct from the creek, and did not suffer from a serious illness that would be defined as luck.

However, although Larry did not witness serious complications, biologically he caused himself some internal bodily harm. For people to be secured in the society, pollution should be minimized starting from homes, businesses, institutions, industries, and even recreational areas(Malabika, 2009).

If some organizations control pollution and others refuse to do the same, it would be hard for the society to live in a healthy manner. No matter how minor a certain pollution may seem to be, any form of pollution should be avoided. The minor pollutions, which are avoided leads to some serious effects to human and animals(Malabika, 2009).

I thank Larry for sharing with me his interesting views concerning the water pollution. I am sure now Larry is convinced that a polluted environment leads to unhealthy society. Never again should he ignore to treat drinking water for the sake of his healthy.

The relationship between community health and the environment is one that cannot be ignored: the environment plays a crucial role in determining the health of a community. Many sectors necessary for human existence such as agriculture, transportation, energy, and industry, influence the environment, and in turn, the health of the community. The health sector is greatly affected by the environment, and community health workers play a crucial role in the prevention approach to health.

Environmental factors such as air pollution, water pollution, soil contamination, climate change, and deforestation are known to have significant health impacts on human beings. Health issues related to hygiene such as respiratory diseases, diarrheal diseases, flus and skin diseases; malnutrition, and deaths due to droughts and famine; accidents such as landslides, cyclones, tsunami, floods, and heatwaves have a great impact on the health systems of an area. These issues affect the physical, mental, and social well-being of individuals and communities.

The relationship between community health and environmental health is complex and multidimensional. For instance, air pollution can cause respiratory diseases such as asthma, lung cancer, and bronchitis. Water pollution can cause diarrhea, cholera, and other water-borne diseases. Climate change can lead to heat stress, vector-borne diseases, and food insecurity. Natural disasters such as floods, landslides, and cyclones can cause injuries, displacement, and mental health problems.

In many communities, community health workers are at the forefront of promoting health and well-being. These workers play a crucial role in educating people on environmental factors that can affect their health. They also help to monitor and report environmental hazards and promote healthy practices such as handwashing, waste management, and safe food practices.

How Environmental Health Impacts Our Quality of Life and Health

 We don’t always see it, but our environment is shaping our health every moment of every day. Where we live, what we eat, and how we interact with the world around us can tip the scales (sometimes literally) between healthy or not. That’s where environmental health professionals, policies, and programs all come into play.

While we tend to think about health in terms of personal choices—like whether we exercise or get vaccinated —a lot of external things can influence how healthy we are, including whether we get sick or injured. Environmental stressors and advantages help shape the kinds of choices you make, or in some cases, make the choices for you.

What Is Environmental Health?

Many people often think of environmental health in terms of clean air and water, but natural environmental forces—including things like global warming—are only one piece of a greater puzzle.

Environmental health is the  public health  field that monitors and addresses those physical, chemical, and biological factors that we might not have direct control over, but can impact our health anyway. For example, if you live in a neighborhood with unsafe sidewalks or polluted air, it’s difficult to get outside and exercise. Similarly, what your home was built with, what insects live nearby, and what food you have access to all can affect your health and the health of your family.

Simply put, environmental health is the area of public health that deals with all the different ways the world around us can impact our physical and mental well-being.

Areas of Environmental Health

Environmental health is one of the largest fields within public health because of the myriad ways external forces can impact how we eat, live, and grow. These forces can be about addressing our natural environment (as in the case for clean water or sanitation), but they can also be the consequence of human beings' actions—including societal norms.

Healthy People 2030's environmental health objectives highlight six key areas that encompass the various ways environmental health is crucial to the health of communities.

Air Quality

Air is non-negotiable for humans. We need it to survive, but we don't always take care to keep it clean, and that can have a significant impact on our health.

Poor air quality has been linked to a wide range of health issues, including SIDS, lung cancer , and COPD . Air pollution is also linked to low birth weight. One study published in 2019 found that babies born to pregnant women exposed to high levels of air pollutants in the early and late states of pregnancy were more likely than their non-exposed peers to be born with a lower birth weight or to be born prematurely.

The Clean Air Act of 1970 sought to change all that. It marked the first time the federal government took responsibility for protecting the air quality for all U.S. citizens by regulating harmful emissions from things like cars and factories. The act was later expanded in 1990 to address acid rain and ozone depletion—and it’s working. In its 2011 prospective report, the Environmental Protection Agency projected that the Clean Air Act would prevent more than 230,000 early deaths by 2020.

Water and Sanitation

According to the Centers for Disease Control and Prevention, an estimated 780 million people worldwide don’t have access to safe drinking water, and a jaw-dropping 1.7 billion (or roughly a fifth of Earth’s population) lack adequate sanitation services like clean bathrooms. The impact of this is staggering. An estimated 2,200 children die every day worldwide of diarrheal diseases linked to improper water and sanitation.

The simple act of filtering and chlorinating water systems in the United States has resulted in significant declines in once-common diseases like typhoid . By one estimate from the UN, for every $1 invested in clean water technologies, the world gains a return of $4.3 associated medical and societal cost savings. Historically, clean water is responsible for the bulk of the decline in childhood mortality in the country.

Toxic Substances and Hazardous Wastes

Toxicology—that is, the area of science devoted to understanding how chemicals and substances can affect people and their surroundings—is an important field in environmental health. Many of the materials needed to advance industries and technology, like heavy metals or even some plastics, can also hurt the human body and even lead to serious medical conditions.

One of the most recent, highly publicized examples of this happening is the Flint water crisis. When news broke in 2015 that the drinking water in Flint, Michigan was loaded with lead, it prompted widespread outrage and fear among families. If kids drink or digest lead , it can lead to long-term health complications, including brain damage, and in the case of Flint, it was the economically disadvantaged kids who were most affected.

More than 40 percent of the population in Flint lives below the poverty line, roughly 2.8 times the national average poverty rate in the United States. The county where the town resides has a history of poor health, ranking 81st out of 82 Michigan counties for health outcomes. The crisis was a prime example of how environmental health issues often hurt those whose health status is already most at risk.

Homes and Communities

We spend the bulk of our time at home, work, or school, so it's important that these places be safe with minimal hazards, as well as be conducive to a healthy lifestyle. When a neighborhood has a lot of violence, for example, families might not go outside to exercise. When roads aren't properly maintained, it can result in more car crashes.

An emerging field in this area of environmental health is that of food access. Many neighborhoods throughout the United States do not have full-service grocery stores nearby. In their absence, residents often have to rely on convenience stores, like those found at gas stations, to buy their groceries. This can be expensive, but most importantly, it can mean fewer or poorer quality options for fresh fruits and vegetables—a vital part of a healthy diet. For families in these areas, it can be a struggle to make healthy choices, exacerbating existing health disparities for low-income and minority populations especially.

In an effort to offset the impact of these "food deserts," environmental health professionals are urging communities to establish public gardens where residents can grow and harvest their own fresh produce, improve access to public transportation to full-service grocery stores and farmers markets, and change zoning laws to incentivize retailers to offer healthier food options .

Infrastructure and Surveillance 

A primary piece of any public health strategy is information. By understanding what the risks are and where environmental health professionals can better deploy resources to prevent or combat them. This includes investigating and responding to diseases—a field called  epidemiology —as well as screening populations for hazards and establishing surveillance programs.

Surveillance activities involve either going out and looking for particular health concerns (active surveillance) or by asking professionals in other fields, such as medicine or agriculture, to alert environmental health agencies when they encounter them (passive surveillance).

An example of this in action is mosquito surveillance and abatement activities. These programs test mosquitoes for certain things, including the presence of dangerous infections like  Zika virus , as well as monitor populations to ensure control measures are working. This information can help health officials know what to watch for in doctors' offices, direct local governments on where and how best to spray for mosquitoes, and alert the public if a mosquito-borne illness is spreading in the area.

Global Environmental Health

In the coming decades, environmental health professionals are bracing for a warmer, wetter climate that will likely prompt or exacerbate threats to our public health across the globe.

As temperatures increase, for example, disease-carrying mosquitoes can live in areas previously too cold for them to survive, upping the number of people impacted by vector-borne illnesses like dengue and malaria . As sea levels rise, whole coastal cities and island nations are at risk for flooding, sending potentially millions of displaced people to crowded areas where diseases can spread quickly.

As extreme weather events become more frequent, health officials anticipate more years like 2017 where back-to-back storms and floods in places like Houston, Florida, and Puerto Rico destroyed homes, facilitated the spread of diseases, and left millions without power.

Protecting the health of the planet is critical to improving and maintaining the health of the entire global population. Even though health outcomes have improved significantly over the past century—in wealthy nations like the United States in particular—environmental hazards and infectious diseases know no geopolitical boundaries. People today are traveling farther and more often than ever before, and conflicts in areas like Syria, Afghanistan, and South Sudan are causing millions to flee their homes.

These increases in cross-border and cross-continental movements have the potential to threaten disease prevention efforts and overextend existing infrastructure. That's why it's crucial that countries look beyond their borders to improve the health of the global population—not just their own.

How You Can Help Protect and Improve Environmental Health

Unlike diet and exercise, many environmental health factors are not something that can be exclusively managed at the individual level. Combatting the risk they pose often takes laws, policies, and programs at the local, federal, and international level.

It’s unrealistic, for example, for everyone to inspect the kitchens of restaurants they frequent or to test their water for heavy metals. That’s why we have trained and qualified food safety inspectors and toxicologists who use rigid, standardized screening and inspection measures to ensure our food and water are safe to consume. It takes a comprehensive and coordinated effort across a vast environmental health system to protect the health and safety of communities throughout the nation and globe.

That being said, there are many things you can do to protect the environmental health and safety of your community and throughout the planet. You can help improve air quality by riding your bike, taking mass transportation, or switching to telecommuting instead of driving a car to and from work.

You can check your own home for radon or lead paint or pipes to prevent exposure to toxic substances. And you can talk with your local governments and businesses about investing in environmental health activities that ensure every neighborhood has access to safe environments to live, work, and play.

Community health is a common term in the field of public health. In essence, community health refers to the discipline, which entails the study and enhancement of the health features or characteristics of biological communities. Noticing that the term community health often has a wider meaning, the usual use of this term focuses on geographical regions instead of people having shared characteristics (Aviles & Filc, 2010). The medical interventions for improving the community health entail improving access to health care, public health campaigns, just but to mention a few. Similarly, the term environmental health is a usual term in the field of public health, which deals with all aspects within the built and natural environment that may influence the health status of human beings. Environmental health involves all the chemical, physical and biological factors that are external to the individual and all the related aspects influencing behaviors. The aforementioned definition excludes behaviors that are not relative to the environment as well as those that have a relationship to cultural and social environment. On the other hand, occupational health is a common term within the discipline of public health. It refers to the identification and control of risks originating from chemical, physical and other workplace hazards with an aim of establishing a healthy and safe working environment. Some of the hazards that occupational health professionals address include electricity, dangerous machinery, chemical agents and solvents, heavy metals such as mercury, loud noise among others (Vonville, 2008). In essence, occupational health programs include training of safety training for workers who handle hazardous wastes. In addition, such programs encompass comprehensive training on environmental restoration for residents living closer to polluted industrial waste sites. All the three branches, community health, occupational health and environmental health are similar to some extent. That is, regardless of their specific areas of focus, all the three branches of public health involve interventions that improve the health status of persons by addressing risk factors. On the contrary, differences among the three disciples exist. For example, community health focuses on the health of a group of people including the levels of well-being with the group while environmental health addresses risk factors external individuals. Occupational health is more different from the other two disciplines because it focuses on improving the health of persons by addressing risk factors in their particular working environments (Vonville, 2008).

Personally, my professional experience attests to the need to maintain high standards of environmental health for the safety of all humanity. While I was at Haiti in 2010 after the occurrence of the earthquake, I witnessed how people suffered from cholera because of using contaminated water. In essence, environmental health can have a serious impact on people’s health, a situation that may threaten human life. In conclusion, it is pertinent to address community, environmental and occupational health issues for the well-being of humanity. The access to health information, provision of safe working environment and campaigns for educating the public on health matters is pertinent to build a healthy and productive society. Therefore, collective responsibility is significant for the realization of health objectives and a quality life for everyone.

The completed works in my portfolio are essential for my development as a healthcare professional and as a learner. These artifacts include tasks on professional roles and values, reports on applied leadership in nursing, assessment on leadership and organizational structures, studies on evidence-based practice in healthcare, and research about community health and nursing interventions for the general population. These studies have ensured that I am knowledgeable about the various concepts in medical care. Therefore, these works also represent my strengths as a healthcare professional.

In addition to this, the artifacts in my portfolio present me not only as a professional but also a learner. As a learner, I have researched numerous concepts in the healthcare sector to gain more knowledge and skills. For instance, I have conducted two studies regarding evidence-based care. In these studies, I explored the effects of oral opioid and nonopioid on acute pain. I have thus gained immense knowledge about this aspect.

Moreover, I am more appreciative of the role of evidence-based practice in healthcare. As a professional, it is my role to explore the various approaches to treatment that improve the quality of medical care and health intervention programs. The tasks in my portfolio, like the tasks on professional roles and values, have facilitated me to achieve this objective.

The works I have completed in the program also present my strengths as a medical care provider. These skills include communication skills, problem-solving skills, analytical strengths, research and evaluation skills, and negotiation skills. These artifacts also indicate that I am knowledgeable about the several crucial aspects of healthcare. Moreover, my works in professional roles and values have described my ethical principles in my career. For a public health nurse, these elements are vital in performing the various duties that form the three main objectives of the profession. A public health nurse must be able to communicate with multiple stakeholders and patients, offer solutions to various health and social challenges in the community, and negotiate and lobby for different policies. Moreover, a health profession must have the necessary skills to analyze multiple policies and health intervention programs. These are the core strengths that will establish me in this profession.

The problem that I have gone through during my advancement in this program is communication challenges with various patients. Some patients find it difficult to understand the different health intervention programs that may be beneficial to them. Moreover, patients come from a variety of backgrounds and cultures. These cultures influence their perception and reception of various healthcare programs. Therefore, one of the primary challenges that I have faced is knowing how to offer healthcare that meets the cultural requirements of these patients.

However, I have utilized my primary strengths and skills to overcome this challenge. As noted above, communication skills are essential in the healthcare profession, and these skills are part of my core professional strengths. I have been able to communicate with several patients and established the proper way of inquiring about the healthcare programs that they prefer and those that they find to be offensive. Communicating with patients is also essential to determine their perception and responses to several health interventions. I have established that patients from multiple backgrounds and cultures appreciate healthcare providers who are keen on their needs and respect their preferences.

A healthcare professional must fulfill several roles, such as scientist, detective, and manager of the healing environment in the professional program. As a scientist, the healthcare professional has to help in the prevention, diagnosis, and treatment of diseases in patients. I have achieved this role by practically working with several patients in multiple settings. Moreover, I have conducted several studies in my role as a scientist on the role of opioid and nonopioid analgesics in pain management.

As a detective, the primary role of a healthcare professional is to analyze and forecast the outcomes of several health intervention programs and the effect of deviations of some aspects in the provision of medical care. I have also achieved this role by evaluating my practice in realistic settings, whereby I have utilized my analytical skills to gauge the effects of specific policies and innovations in medical care provision. I also examined the consequences of my decisions in various ethical dilemmas like when I handled a patient who requested personal help which conflicted with my ethical principles.

In addition to this, I have fulfilled the role of a manager of the healing environment in my program by creating suitable conditions for patients that meet their various preferences, including the social, cultural, political, and economic aspects. Through this process, I have worked to reduce environmental and internal stressors that may affect the healing process of my patients. Furthermore, I have analyzed several elements that contribute to the treatment process, such as privacy, waiting times, and spacing in the healthcare center, and formulated ways to address them. This factor also reduced the environmental stressors for patients.

Through all these processes, I have grown both personally and professionally. As a healthcare professional, I have grown to understand the various factors that affect the treatment process to patients and the patient’s perception of these factors. This process has enabled me to offer better care to patients and improve their treatment outcomes. In addition to this, I have developed several skills that are necessary for the development of health intervention programs and policies. For instance, I now understand the benefits and necessity of using evidence-based practices in healthcare provisions. Moreover, I have evaluated the quality of my care through analyses of realistic scenarios.

Quality and safety

From my studies on professional roles and values, I have defined quality of care as the level to which the healthcare providers meet the needs of the patient by offering medical care, which improves the health outcomes of patients. Safety refers to this component of improved health outcomes. The safety aspect of healthcare programs may also refer to programs that do not endanger the life or wellbeing of individuals. Therefore, quality and safety are interrelated components that form the foundation of healthcare programs.

This program contained tasks that required me to evaluate professional roles and values in healthcare. In these tasks, I conducted several studies, such as reviewing the Novice to Expert Theory, the importance of sanitary conditions in treating patients, and the roles of nursing associations and regulatory agencies. Through these studies, I have understood the aspects of quality of treatment procedures and developed the definitions.

The works in this program that enable me to define the quality of healthcare and safety are my tasks in the roles and values of medical professionals. These studies have helped me to evaluate the different elements in healthcare programs and their effects on medical care provision. Moreover, I have also conducted several studies on evidence-based practices in healthcare, which have informed me regarding the aspects of quality and care and have assisted me in formulating a suitable definition.

Evidence-based practice refers to the development and use of healthcare programs and policies based on evidence from scientific studies. These studies must encompass the use of systematic data and information. Therefore, evidence-based practices focus on the quality of healthcare programs and the safety of patients by ensuring the health interventions that healthcare providers offer to patients have scientific backing. Evidence-based practice helps to avoid adverse effects on patients’ health by using unproven interventions. Moreover, evidence-based practices ensure that healthcare interventions are safe for all patients, including in the aspects of the social determinants that affect them.

The program has been influential in shaping my definitions of evidence-based practices. In my first evidence-based practice task, I evaluated the effect of different analgesics on acute extremity pain in the emergency department. In contrast, my second task on evidence-based research considered the aspect of the nurse to patient staffing ratios. These assessments have given me a background in the study of these practices, which has informed my definition. I have, therefore, gained significant knowledge regarding these facets of the healthcare profession. I am also more capable of formulating a definition of evidence-based practices according to my background analyses.

The evaluation of primary research is essential in the utilization of evidence-based practices and applying these concepts to my nursing practice. The most critical elements in these assessments are establishing the accuracy of data, determining the differences between research and improvement of quality aspects, and analyzing primary and secondary research and their implications.

Reflection on my understanding of evidence-based practices

To establish the relevancy and accuracy of data, one assesses whether the researchers have disclosed any bias in the study. Moreover, an assessment of the methodology of the study, including the sampling method and data collection process, determines the study’s accuracy. One can also resolve the accuracy and relevancy of the study by comparing the results between different studies.

The significant difference between quality improvement and research is that the latter applies the methodology of data collection and data analysis to draw inferences relating to various elements. However, quality improvements do not rely on methods for analysis but instead focuses on improving patient outcomes by using systematic procedures from completed research. Therefore, one can distinguish these two facets by examining the methodologies in the studies. Research is applicable in the development of new processes or interventions, while quality improvement is applicable in using analysis from research to formulate procedures and responses.

Furthermore, the difference between primary studies and secondary research is that the former involves the firsthand collection and analysis of data. In contrast, secondary research consists of studying research which other researchers have already done and concluded. Primary data is often applicable when there is no specific information from completed studies, and therefore, a researcher has to analyze data to develop the needed information. The program has enabled me to determine these aspects and differences by guiding me through the processes of using various studies for different requirements.

Applied leadership

Applied leadership in healthcare is the process by which nurses use their learned leadership skills to communicate with other individuals within their teams professionally. Nurses and healthcare providers must effectively communicate with their teams to ensure they coordinate the healthcare delivery process to the patients. In addition to this, applied leadership is also essential for professional collaboration. Applied leadership in nursing follows the core principles of assessing the vulnerabilities of patients to diseases, using evidence-based practices for continuous improvement, and improving the efficiency and coordination of healthcare delivery. I will, therefore, employ the leadership skills I have gained throughout the program to offer quality care to all my patients.

I have conducted several studies in the program regarding applied leadership, including one on organizational systems and quality leadership. Through this task, I learned several aspects relating to medical care, like the role of the National Database of Nursing Quality Indicators (NDNQI). The (NDNQI) has helped me to define the various facets of applied leadership by establishing the professional mission statement of medical professionals. The program also provides the qualities and principles that are essential in applied clinical leadership. Moreover, the task of quality leadership has also taken me through various practical scenarios where I have applied my leadership skills in my team for the patients’ benefits.

Several items that form the basis of my definition of applied leadership include the lists of readings that I have undertaken, reports of various studies in which I have participated, and transcripts of my clinical rotations. My clinical rotations form the most significant elements of my practical experiences in the nursing program. Community and population health

My portfolio also includes a report on the elements of community health nursing. This study influenced my definition of these facets of healthcare by offering me insights regarding the various elements of public health. In my interpretation, public health encompasses both community and population health. Moreover, the program also equipped me with the necessary skills for community and population health provision through my practical experiences when working with various communities and populations. I gained these experiences through my community health nursing task. The program also required me to assess multiple studies regarding the elements of public health provision.

The items in my portfolio which have guided my understanding of population and community health are my reports regarding the studies on public health, and particularly on the facets of community health and population health. These artifacts support my definition by analyzing the concepts of community health and population health. Moreover, these studies also offer insights regarding the various elements of these health intervention programs. These perspectives relate to the approaches of community health programs and their benefits and have shaped my understanding of community health.

The main lesson from my community health task is that communities comprise of persons with different health needs. These individuals also face social determinants that influence their health and health needs in different ways. Therefore, community health programs must consider these factors in the formulation of health intervention programs for the community. By so doing, the healthcare providers will be able to target the multiple health needs of the individuals in the community for better outcomes. Moreover, healthcare providers must partner with various members of the community to ensure they can effectively diagnose the different healthcare requirements of the community members.

In my initial community diagnosis, I had established that the main social determinants that influenced the community’s health are poor nutrition and unstable economic conditions. I also determined that these social determinants led to conditions such as malnutrition for some of the community members, while others suffered from similar ailments such as iron deficiency. However, after working with the community and several leaders of the population, I established that the community did not have a challenge with its economic conditions. The main factor for the community’s health needs was that they lacked the necessary information regarding the proper ways to prepare healthy diets.

Summary of American Museum of Natural History

The American Museum of Natural History (AMNH) professional course focuses on the elements of genetics, genomics, and genetics. These aspects offer promise for the future of medicine in the treatment of multiple diseases and the development of vaccines. Essays in Health, Development and the Environment

As multiple regions in the global south urbanize and transform, their social-environmental challenges also reshape. Climate change and ecological degradation intertwine with these processes in ways that have an uneven impact on people and firms with various degrees of vulnerability. In this dissertation, I look at such issues through the lens of sustainable development, with a regional emphasis on Mexico. Standard economic analyses of environmental policy focus on either reducing pollution externalities through mitigation or reducing the harms from exposure by encouraging adaptation. In practice, these issues are both critical, particularly when looking at the health effects of local air pollutants, which can be acute, and policymakers often pair information provision with short and long-run mitigation actions. In Chapter 1, I explore whether, in the context of the Mexico City air quality alert program, information policy is more effective when paired with mitigation. I find that the policy did not improve air quality or health outcomes until the mitigation component, which limited transport emissions, was introduced. I also use sensor-level traffic data, geo-tagged accident reports, and search data as a measure of awareness of the policy to unveil the mechanisms through which considerable short-run improvements in air quality and health are achieved after issuing an alert. I find that the alert reduces car usage even before the driving restrictions enter into place, suggesting that, due to an increased awareness of pollution, people reduce their trips. Chapter 2 studies the effects of regional exposure to extreme temperatures on credit delinquency rates for firms in Mexico. Our exposure variable is defined as the number of days in a quarter that minimum and maximum temperature are below 3°C and above 36°C, respectively, which correspond to the bottom 5 percent and top 5 percent of daily minimum and maximum temperature distribution in the country. We find that extreme temperatures increase delinquency. This effect is mostly driven by extreme heat, and it is concentrated on agricultural firms, but there is also an effect on non-agriculture firms. The impact on non-agricultural firms seems to be driven by general equilibrium effects in rural areas. Chapter 3, provides the first estimation of child penalties in the Mexican labor market. Using an event study approach and an instrumental variable as a robustness check, we estimate the impact of children on employment and wages, unpaid labor, and transitions between informal and formal sectors. We are the first to show that a child’s arrival significantly affects mothers’ paid and unpaid work, and it impacts members of the extended family unevenly, reinforcing traditional gender roles. While low- and middle-income women account for most of the effect of childbirth on wages, all mothers increase time spent on unpaid work.

Environment and health: an overview

In the 1980s biologists began to observe startling declines in frog populations around the world, even in isolated and relatively pristine environments. It appears that no single factor is responsible. Rather, the health and reproductive success of amphibians is being damaged by an increase in the intensity of ultraviolet (UV) light (because of thinning of stratospheric ozone), traces of globally distributed toxic chemicals, competition from introduced predator species and infections caused by virulent fungi and bacteria. 1 , 2 The declining health of frogs, birds and thousands of other organisms may be one of the clearest indications of environmental threats to human health. Although local environmental tragedies of climate change, species extinction and deforestation have marked every period of human history, today's environmental degradation is rapidly creating an unprecedented global crisis driven by population growth and industrialization. 3 , 4 , 5 , 6 , 7

For the first time, human beings are altering the basic operations of the Earth's atmosphere, geosphere and biosphere. In a recent essay, 4 prominent biologists noted with concern that "human alteration of Earth is substantial and growing. Between one-third and one-half of the land surface has been transformed by human action; the carbon dioxide concentration in the atmosphere has increased nearly 30% since the beginning of the Industrial Revolution; more atmospheric nitrogen is fixed by humanity than by all natural terrestrial sources combined; more than half of all accessible surface fresh water is put to use by humanity, and about one-quarter of bird species on Earth have been driven to extinction." 8 In 1992 the World Scientists' Warning to Humanity 9 was endorsed by more than 1600 scientists from 70 countries, among them 104 Nobel laureates, including most of the science prize recipients. The warning cited clear evidence of a growing environmental crisis. 9

Ten years ago Alexander Leaf wrote about the potential effects of global environmental change on human health. 10 This essay in CMAJ introduces a series of articles that continue Leaf's initial exploration. In each article, the authors will present a brief state-of-the-science review of their topic, an interpretation of the problem, and suggestions for medical and public health responses. The intent of the series is to examine the links between environmental change and human health and to suggest programs and policies that will protect both health and the environment. These essays do not address environmental hazards for which the association with disease is well understood, such as environmental lead poisoning, particulate and ozone air pollution, radon and tobacco smoke. Instead, these papers will focus on global environmental changes precipitated by human activity and their likely role in emerging health problems.

From a biomedical standpoint health is viewed as an attribute of the individual. The fields of medicine and public health acknowledge environmental causes of illness and assign risk to specific exposures. In the past decade, biologists, ecologists and physicians have developed the concept of ecosystem health. This idea recognizes that humans are participants in complex ecosystems and that their potential for health is proportional to the health of the ecosystem. 11 An ecosystem-based health perspective takes into account the health-related services that the natural environment provides (e.g., soil production, pollination and water cleansing) and acknowledges the fundamental connection between an intact environment and human health. 12

Environmental degradation exaggerates the imbalance between population and resources, increases the costs of development, and worsens the extent and severity of poverty. Population growth and the "corporatization" of agriculture and forestry have forced poor people onto land that is the least productive and ecologically the most fragile. In crowded or poor countrysides, people often abandon traditional and sustainable land use practices in favor of short-term survival strategies such as farming on steep slopes and living in areas threatened by flood or drought. The need for farmland, fuel wood and timber for export results in deforestation, which in turn increases soil erosion, flooding and mud slides and reduces agricultural productivity. In short, interactions between poverty, population growth and environmental degradation impede sustainable economic development and worsen population health. 13

The problems resulting from environmental change pose new challenges for traditional public health science. 14 The health effects of global change are often indirect and difficult to assess, and the quality of evidence for the health- related outcomes of global environmental change varies widely. 7 For example, the prevalence of malaria has increased worldwide, but no clear relation to climate change has been established. Similarly, exposure to UV light (especially UVB) increases skin cancer and cataract formation, but large studies across geographical areas with different levels of UV exposure have not been performed. Furthermore, the health science necessary to understand global environmental change is increasingly interdisciplinary and requires collaboration among meteorologists, chemists, biologists, agronomists, biologists and health scientists, over long periods. Organizing and funding such science is difficult. Finally, the science of global change frequently relies on computer models to suggest the direction of change, but politicians and policy-makers are loath to commit resources to predicted but unproven future outcomes.

It is important for scientists to anticipate the potential consequences of environmental change. 15 Serious environmental problems are often unknown or unrecognized. The stratospheric ozone hole produced by chloroflurocarbons, although anticipated, was discovered by accident. 16 At the time of the first major international conference on the environment, held in Stockholm in 1972, global warming, acid rain and tropical deforestation were not recognized as major problems. Explanations of the decline in amphibian populations, cancer outbreaks in fish and the bleaching of coral reefs are still inadequate today. Furthermore, change in natural systems may be sudden and nonlinear. For example, fish populations that have remained stable during long periods of intense harvesting may suddenly collapse.

Global environmental issues at this special moment in history are unique in their scope and consequences, and discussion of them may be emotionally and politically charged. 17 Global change may seem so remote from our daily lives that we become indifferent to the litany of environmental apocalypse. We may not perceive the actual degradation of the Earth. 18 Some of us distance may ourselves from the discussion because we find it frightening or overwhelming As a result, policy-makers and politicians are not pressed to confront the consequences of the continuing expansion of human enterprise. For example, climate change produced by the accumulation of greenhouse gases, primarily carbon dioxide, seems increasingly certain. The 1995 second report of the Intergovernmental Panel on Climate Change concluded that "man's impact on climate is discernible," and many climate scientists believe that we are already experiencing global warming effects. 19 But it is difficult for us to acknowledge that industrial carbon dioxide — invisible, odourless and nontoxic — is a pollutant. International political leadership has only recently begun to seek solutions to the global issues of climate change, toxic pollution, species loss and deforestation.

Two recent developments have drawn renewed attention to the health risks of persistent organic pollutants (also known as POPs): the identification of medical waste as a significant source of toxic pollution and the emergence of the new toxicological field of endocrine disruption. Medical waste incineration is a major source of the dioxin and mercury released into the environment. Almost all humans have measurable residues in their tissues of chlorinated hydrocarbon chemicals, including pesticides, polychlorinated biphenyls (PCBs) and dioxin. In some cases these levels approach the threshold of public health concern. 20 Pressured by advocacy organizations, the health care industry has begun efforts to better manage medical materials and waste. The second development is the emerging toxicological field of endocrine disruption. Endocrine disruptors are a class of chemicals, including many of the persistent organic pollutants, that imitate or block hormones. These chemicals produce a variety of reproductive and neurodevelopmental disturbances in wildlife, laboratory animals and humans, often at very low doses. Endocrine disruption, currently the object of renewed study by government, industry and academia, offers a new toxicological paradigm that may supplement carcinogenesis as the outcome of concern. 21 , 22

Public concern about environmental degradation in both rich and poor nations is developing into a broad environmental health movement. 23 But to argue, as some do, that the quality of human existence is improving because life expectancy is increasing and child mortality is decreasing in many parts of the world is to miss what McMichael has called the "essential newness" of environmental change. 14 The carrying capacity of the Earth may appear adequate at this moment in history, particularly for those of us in affluent countries. Economic development and improved access to public health programs have produced expected improvements in less developed countries. Although the world's population has increased fourfold in the last 150 years, the food supply has kept pace. But can we support another approximate doubling of the population by 2050, from 6 billion to 10 or 12 billion, the high-fertility forecast by the United Nations? 24 Will the food supply remain adequate? What are the health consequences of global warming and climate change? 25 What are the consequences of loss of biodiversity, forests and marine life? 26 Science has only begun to address these questions.

To protect the health of populations we must develop systems of food, energy and industrial production that can be sustained over generations. We also need value systems of stewardship, precaution and prevention to guide environmental protection and health promotion. Finding solutions to the threats posed by environmental change is the major health challenge of the next century. 27 , 28

Biden Tried to Show Solidarity With East Palestine. He Found Divisions Instead.

The president visited the Ohio town over a year after a devastating train derailment. He faced a divided community anxious over the long-term health consequences of an environmental disaster.

President Biden traveled to East Palestine on Friday to show solidarity with a community still reeling more than a year after a Norfolk Southern train derailed, spilling a toxic mess across this small Ohio town near the border with Pennsylvania.

But what he found when he got here were divisions on every corner.

On the main street, protesters accused the White House of neglect and asked why it had taken Mr. Biden so long to visit. More than 100 Trump supporters, some of whom had driven in from out of state, held up “Impeach Biden” signs. And still others said they were sick of their town being used as a political prop and wanted answers about their long-term health.

Mr. Biden alluded to some of those differences during remarks near the crash site, saying that “it doesn’t matter if you’re a Democrat or Republican or independent.”

“What matters is we’re all Americans,” Mr. Biden continued. “We look out for one another. We leave no one behind. And we come back stronger than before.”

Mr. Biden also said the National Institutes of Health would issue six grants to research universities to study “the short and long term impacts” of the toxic spill — a key concern among residents, many of whom complain of health problems since the accident.

Tens of thousands of tons of contaminated solid waste and millions of gallons of wastewater have been shipped out of East Palestine since the derailment, the Environmental Protection Agency said. Ohio declared the drinking water safe just weeks after the crash.

“The president has no concerns with drinking the water in East Palestine,” Karine Jean-Pierre, the White House press secretary, said this week. She was responding to a reporter who mentioned that President Barack Obama had drunk filtered tap water in Flint, Mich. , in 2016 after a lead contamination crisis in the city.

During Friday’s visit, Mr. Biden took a sip of coffee brewed with tap water while visiting a local candle-making workshop and also had a cup of water beside him as he met with the business owners and local residents.

Still, the fact that it took Mr. Biden a year to visit East Palestine, a town of 5,000 people, bothered many people in the community, particularly because he had promised to do so soon after the derailment.

Republicans and many residents have said his absence was a sign of disrespect .

“It’s sad he waited so long,” said Kathleen Unkefer, 68, a lifelong resident of East Palestine and florist who watched the different rallies through the window of the Flowers Straight From the Heart shop. “I just feel it’s all political. It’s an election year.”

The White House has said Mr. Biden wanted to go at a moment when his visit would not be disruptive. And Biden administration officials have defended the federal response, saying the government has sent a steady flow of federal resources to the community, and deployed hundreds of people to assess the risks in the days after the spill.

The administration has said Norfolk Southern should be held accountable for the harm caused by the derailment.

“While there are acts of God, this was an act of greed that was 100 percent preventable," Mr. Biden said on Friday.

Mr. Biden has not approved a presidential disaster declaration, as Ohio’s governor requested last year, and as many residents were hoping he would announce on Friday. But he assured residents that the federal government would stay invested in its recovery.

“What they do not make whole, what they cannot make whole, the government will make whole,” he added of Norfolk Southern. “We have an obligation.”

Norfolk Southern has said the cost of the derailment cleanup, legal costs and assistance to the community will be more than $800 million. The company also has committed more than $100 million toward costs including direct payments to residents and reimbursements for emergency medical workers.

“We’re committed to helping them make it an even more vibrant, thriving place,” Alan H. Shaw, the chief executive of Norfolk Southern, said in a statement.

The train was carrying more than 700,000 pounds of vinyl chloride, a carcinogen used to produce pipes, furniture and packaging, when it derailed. Emergency responders set the chemicals ablaze during a so-called controlled burn to avert a wider explosion.

On Friday, Mr. Biden ignored shouted questions about the timing of his visit after his official remarks.

Presidential visits to devastated communities can sometimes provide the opportunity for Democrats and Republicans to play down their differences and focus on showing empathy. Local leaders have often commended Mr. Biden on such occasions.

But Mr. Biden, who ran for president on a pledge to unite the nation, confronted a community that is a microcosm of the politically divided country.

Former President Donald J. Trump, Mr. Biden’s likely opponent in this year’s presidential race, visited East Palestine in the weeks after the derailment. Hours before Mr. Biden arrived on Friday, the Trump campaign issued an advertisement titled “Too Little, Too Late.”

Even the invitation for Mr. Biden to visit from the mayor, Trent Conaway, carried a hint of the division. Mr. Conaway has criticized Mr. Biden for allowing Mr. Trump to visit before him.

Jaime Nentwick, a 37-year-old organizer at an Ohio environmental justice nonprofit, said the situation in East Palestine has been politicized.

“It’s been a prop since the derailment,” she said. “The most important thing in order to give the residents some kind of heightened hope, to restore that, is to give an actual factual reason as to why he hasn’t been here in a year.”

Ann arbor creeps closer to environmental goals with electric vehicle milestone, 25% of light-duty vehicles now evs.

ANN ARBOR, Mich. – The City of Ann Arbor has hit a new milestone with its municipal fleet as it creeps closer to its sustainability and environmental goals.

As of the end of 2023, 25% of the city’s light-duty vehicles -- 53 to be exact -- are now electric, authorities said. Those EVs include vans, SUVs, pickup trucks and hatchback models.

Officials said the milestone helps the city advance its Green Fleets Policy, which aims to reduce fossil fuel usage to help public health, the environment and City operations costs.

More EVs are expected to join the city’s fleet by the end of the 2024 fiscal year, bringing the total number of EVs up to 35% of the fleet.

“The City of Ann Arbor and its collaborators have worked extensively to reduce greenhouse gas (GHG) emissions from transportation in the community. Transportation-related emissions account for approximately 30% of GHG emissions in the US, and around 26% of emissions here in Ann Arbor,” reads a city notice.

Adding EVs to the city’s fleets is also part of the A2ZERO Plan, which guides the city towards carbon neutrality. Two strategies on the plan highlight reducing greenhouse gas emissions by electrifying city appliances and vehicles and reducing the number of miles those vehicles travel.

Heavy-duty vehicles in the city fleet are also slowly being replaced with electric vehicles. This includes an electric bucket truck, electric forklifts, electric utility vehicles and electric outdoor equipment.
