Groundbreaking research could revolutionize how bowel cancer is treated

New research led by a team of scientists from Queen's University Belfast and the Cancer Research UK (CRUK) Scotland Institute in Glasgow have made a series of groundbreaking discoveries into tumor biology that may be used to deliver a more effective personalized medicine approach for patients with bowel (colorectal) cancer.

Bowel cancer is the fourth most common cancer in the UK, with around 42,900 new bowel cancer cases and around 16,600 bowel cancer deaths in the UK every year, a statistic that highlights the need for new ways to treat patients with this aggressive disease.

The new study published in Nature Genetics , used a unique and innovative approach developed in Belfast, to identify a set of previously unseen molecular patterns in tumor tissue that provides new information related to treatment response and risk of disease progression.

These remarkable results mean that clinicians and scientists can now draw more information from a patient's tumor tissue which may lead to better treatment options.

Previously, the most common approach for identifying groups of tumors based on their biological signaling, known as molecular subtyping, used information about how active individual genes are within tumor tissue.

The subtypes identified using this method have shaped understanding of cancer development, progression and response to therapy over the last decade and served as the basis for numerous clinical trials and pre-clinical studies.

Dr. Philip Dunne, Reader in Molecular Pathology from the Patrick G Johnston Center for Cancer Research at Queen's University Belfast and senior author of the study explains, "While looking at patterns across individual genes has revealed remarkable insights into cancer signaling, advances in laboratory research over the last decade have shown that assessing activity in groups of closely related genes in combination can provide new understanding that isn't apparent when using the traditional single gene approaches."

Given the potential value of this new pathway approach, researchers from Queen's University Belfast, the CRUK Scotland Institute, University of Zurich, University of Oxford alongside a multinational collaborative group proposed a new data-driven method for reclassification of bowel cancer, which has been published in this new groundbreaking study.

By assembling genes into biological pathways prior to the development of molecular subtypes, the team rearranged tumors into a series of new groups, based on activity across a complex network of cancer-related signaling; all of which appears to be critical in predicting how well a tumor will respond to different treatments such as chemotherapy and radiotherapy.

To ensure that scientists and clinicians around the world can immediately access these data and test this new subtyping approach, the team have released a freely available classification tool that allows the Belfast-developed approach to be used on tumor samples in any research lab.

Dr. Sudhir Malla, Postdoctoral Research Fellow from the Patrick G Johnston Center for Cancer Research at Queen's University Belfast and first author of the study said, "By developing an unrestricted classification tool for the cancer research community, it means that researchers from around the world can reproduce our finding on data emerging from their own collections of tumor samples, to identify biological pathways that cancer cells switch on or off to control their movement, growth and response to their environment."

Professor Owen Sansom, Director of the Cancer Research UK Scotland Institute in Glasgow, who co-authored the study said, "The research presented today in Nature Genetics exemplifies the value of collaborative research between scientists and clinicians. Studies such as this are essential to enable us to understand the complexities of a patient's tumor and will be used in our pre-clinical laboratories to identify novel treatments specifically targeting the biological patterns we have identified."

Dr. Sam Godfrey, Research Information Lead at Cancer Research UK, said, "Cancer is perhaps the most complex disease we face, and no single treatment will ever beat it."

"Research like this could lead to better and more precise treatments for the thousands of people diagnosed with bowel cancer every year in Scotland."

The team is now applying their new subtyping approach on tumor samples derived from clinical trials run in the UK, to build the essential clinical evidence needed before the new method can be used to make clinical decisions about which treatments a patient should be offered.

Discrete Choice, Complete Markets, and Equilibrium

This paper characterizes the allocations that emerge in general equilibrium economies populated by households with preferences of the additive random utility type that make discrete consumption, employment or spatial decisions. We start with a complete markets economy where households can trade claims contingent upon the realizations of their preference shocks. We (i) establish a first and second welfare theorem, (ii) illustrate that in the absence of ex-ante trade, discrete choice economies are generically inefficient, (iii) show that complete markets are not necessary and a much smaller set of securities decentralizes the efficient allocation. We illustrate the relevance of these results in several canonical settings and for measuring how welfare changes in response to changes in prices.

Science sleuths are using technology to find fakery and plagiarism in published research

Allegations of research fakery at a leading cancer center have turned a spotlight on scientific integrity and the amateur sleuths uncovering image manipulation in published research.

Dana-Farber Cancer Institute, a Harvard Medical School affiliate, announced Jan. 22 it’s requesting retractions and corrections of scientific papers after a British blogger flagged problems in early January.

The blogger, 32-year-old Sholto David, of Pontypridd, Wales, is a scientist-sleuth who detects cut-and-paste image manipulation in published scientific papers.

He’s not the only hobbyist poking through pixels. Other champions of scientific integrity are keeping researchers and science journals on their toes. They use special software, oversize computer monitors and their eagle eyes to find flipped, duplicated and stretched images, along with potential plagiarism.

A look at the situation at Dana-Farber and the sleuths hunting sloppy errors and outright fabrications:

WHAT HAPPENED AT DANA-FARBER?

In a Jan. 2 blog post , Sholto David presented suspicious images from more than 30 published papers by four Dana-Farber scientists, including CEO Laurie Glimcher and COO William Hahn.

Many images appeared to have duplicated segments that would make the scientists’ results look stronger. The papers under scrutiny involve lab research on the workings of cells. One involved samples from bone marrow from human volunteers.

The blog post included problems spotted by David and others previously exposed by sleuths on PubPeer , a site that allows anonymous comments on scientific papers.

Student journalists at The Harvard Crimson covered the story on Jan. 12, followed by reports in other news media. Sharpening the attention was the recent plagiarism investigation involving former Harvard president Claudine Gay, who resigned early this year .

HOW DID DANA-FARBER RESPOND?

Dana-Farber said it already had been looking into some of the problems before the blog post. By Jan. 22, the institution said it was in the process of requesting six retractions of published research and that another 31 papers warranted corrections.

Retractions are serious. When a journal retracts an article that usually means the research is so severely flawed that the findings are no longer reliable.

Dr. Barrett Rollins, research integrity officer at Dana-Farber, said in a statement: “Following the usual practice at Dana-Farber to review any potential data error and make corrections when warranted, the institution and its scientists already have taken prompt and decisive action in 97 percent of the cases that had been flagged by blogger Sholto David.”

WHO ARE THE SLEUTHS?

California microbiologist Elisabeth Bik, 57, has been sleuthing for a decade. Based on her work, scientific journals have retracted 1,133 articles, corrected 1,017 others and printed 153 expressions of concern, according to a spreadsheet where she tracks what happens after she reports problems.

She has found doctored images of bacteria, cell cultures and western blots, a lab technique for detecting proteins.

“Science should be about finding the truth,” Bik told The Associated Press. She published an analysis in the American Society for Microbiology in 2016: Of more than 20,000 peer-reviewed papers, nearly 4% had image problems, about half where the manipulation seemed intentional.

Bik’s work brings donations from Patreon subscribers of about $2,300 per month and occasional honoraria from speaking engagements. David told AP his Patreon income recently picked up to $216 per month.

Technology has made it easier to root out image manipulation and plagiarism, said Ivan Oransky, who teaches medical journalism at New York University and co-founded the Retraction Watch blog. The sleuths download scientific papers and use software tools to help find problems.

Others doing the investigative work remain anonymous and post their findings under pseudonyms. Together, they have “changed the equation” in scientific publication, Oransky said.

“They want science to be and do better,” Oransky said. “And they are frustrated by how uninterested most people in academia — and certainly in publishing — are in correcting the record.” They’re also concerned about the erosion of public trust in science.

WHAT MOTIVATES MISCONDUCT?

Bik said some mistakes could be sloppy errors where images were mislabeled or “somebody just grabbed the wrong photo.”

But some images are obviously altered with sections duplicated or rotated or flipped. Scientists building their careers or seeking tenure face pressure to get published. Some may intentionally falsify data, knowing that the process of peer review — when a journal sends a manuscript to experts for comments — is unlikely to catch fakery.

“At the end of the day, the motivation is to get published,” Oransky said. “When the images don’t match the story you’re trying to tell, you beautify them.”

WHAT HAPPENS NEXT?

Scientific journals investigate errors brought to their attention but usually keep their processes confidential until they take action with a retraction or correction.

Some journals told the AP they are aware of the concerns raised by David’s blog post and were looking into the matter.

PROVIDENCE, Rhode Island: The field of cancer biology is a mess. Signs of trouble emerged years before the most recent scandal, in which investigators found evidence of data manipulation in a slew of high-profile papers from the Harvard-affiliated Dana Farber Cancer Institute.

It’s the latest crisis in academic research, where there’s a clear need for better quality control - a tighter filter than peer review. Some researchers suggest that artificial intelligence (AI) could help point out which papers need closer scrutiny. 

But to understand what’s going on, we have to understand how we got here. A decade ago, some research watchdogs started raising alarms after scientists found fewer than half of “landmark” pre-clinical cancer studies - those in top journals - could be replicated. 

In 2021, a similar evaluation found that hype is the norm. Researchers found they could only reproduce 50 out of 193 experiments. And in those that did replicate, the second try showed much smaller effect sizes - only 15 per cent as big as what had originally been claimed. 

These are the kinds of experiments in test tubes or in mice that determine which treatments get tested in people. They also influence how trial subjects are informed about risks and benefits. So the results affect the lives of real people .

Bigger claims with flimsy evidence.

While evidence of data tampering - what the Dana Farber scientists are accused of - is a different problem from irreproducible results, both stem from the same root causes. Scientists gain fame and fortune by obtaining flashy, potentially high-impact findings, but people benefit from findings that are solid and reproducible. We also benefit from findings that show which treatments are unlikely to work, though these are hard to get published.

As Nobel winner William Kaelin warned me back in 2017, biomedical researchers have started making bigger claims with flimsier evidence. (He’s also at Dana Farber, but his work hasn’t been named in this current scandal.) 

Scientists are allowed to make mistakes, of course. But they are supposed to present their data exactly as they measured it. Any graphs are supposed to represent that data as measured. Adding, subtracting or changing data without explanation is usually considered an act of fraud. 

While the case is still being investigated, Dana Farber plans to retract six papers and issue corrections in many more. It’s possible that the problems in some of the papers might have been accidental, but there are an awful lot of them - and such errors would still cast doubt on the findings. 

Data manipulation is all too common, said Ivan Oransky, co-founder of the blog Retraction Watch. “The part that worries me is we’re going to continue treating this like this weird anomaly, which it isn’t.” 

A study that doesn’t replicate, on the other hand, might have been done according to all the rules, but the conclusions aren’t ones you’d want to bet the lives of cancer patients on. The researchers might have misinterpreted their data or the experiment might work only under very specific conditions.

So why hasn’t peer review prevented the publication of weak results and outright fraud? For one, many papers don’t include their raw data, making fraud hard to spot.

But at a deeper level, peer review isn’t the quality control measure many people assume. Some historians trace peer review back to 1830, when English philosopher William Whewell proposed it for papers to be published in a new journal, the Proceedings of the Royal Society of London. In the first attempt, Whewell himself took on the job but couldn’t agree with a second reviewer, thus ushering in a long tradition bemoaned by scientists the world over. 

Reviewers often have the expertise to evaluate 90 per cent or 95 per cent of a paper, said Brian Uzzi, a social scientist who studies problems with replication at the Kellogg School of Management at Northwestern University. “You'll leave that last 5 per cent hoping that the other reviewer is going to pick up on it. But maybe the other reviewer is doing the same thing,” he said. Reviewers are also often pressed for time, overwhelmed by other review requests and their own research obligations.

Uzzi found that in social science, where there’s been a longstanding reproducibility crisis, machine learning can flag the papers most likely to fail attempts at replication. He used data on hundreds of attempted replications to train a system that he then tested on 300 new experiments for which he had replication data. The machine learning system was more accurate than individual human reviewers, as well as inexpensive and almost instantaneous. 

Perhaps such systems could help human experts do more to flag sloppy and dishonest work by taking a first pass. It could also help direct overworked reviewers and journal editors away from the famous scientists and institutions who tend to get the most attention and toward important findings by lesser-known teams.

Scientists already create a flood of new research papers, so it wouldn’t hurt to add a new layer of quality control and put more time and money into separating good papers from bad. Otherwise, we will be paying for all that bad research - not only with our tax dollars, but with our health.
