If you’re a teacher, former teacher, or simply have years of experience caring for children — and an entrepreneurial streak — you might have considered starting your own day care center, either from home or in a dedicated facility. And at an expected job growth of 7% over the next decade, working in child care is a stable career choice. So, if you’re seriously wondering how to start a day care center, you’ve come to the right place.

As is the case starting a business in any industry, however, your passion for your craft alone — or, in this case, your students — isn’t quite enough to ensure that your day care business is copacetic, either financially or legally. You’ll need to do a good amount of due diligence when looking into how to start a day care business, paying special attention to licensing requirements, and ensuring that your facility and program aligns with your state’s health and safety codes.

If you’re a pro at educating, training, supporting and generally corralling large groups of small children, you should be pretty unperturbed by the work it takes to start a day care business. Here’s what you need to know.

Although every path toward starting a business looks different for every business owner, there are a few steps that every aspiring day care owner needs to think about when it comes to how to best start a day care.

1. Decide what kind of day care business you want to start

Before you can even get to writing a business plan for your day care business, you need to decide what kind you want to open. Look into whether you want to start a day care business at home, or whether you want a more commercial facility — there might be different permits required in your municipality depending on which you choose. You also need to decide what age groups you want to focus on.

Next up, you'll have to decide on a business entity, which will have huge effects on the way you're taxed and how you operate your day care business. Will you have partners or open your business on your own? Additionally, you'll want to consider whether you want a business entity that offers you some protection. Making your day care center a limited liability company might be a good move, since the business will take on liability should anything go wrong.

You might also decide to buy into a day care franchise opportunity. This option will significantly streamline the steps you need to take in opening your day care.

2. Write a day care business plan

This is a step you have to take when starting any business and it can be quite a bit of work, especially for anyone looking to get moving quickly on their business. When you start writing a business plan, start with an outline of all the things you want to include.

Your plan should include a summary, an overview of the company, a market analysis that includes an assessment of the need for a business like yours, a marketing and sales plan, and a financial plan along with financial projections. Don't worry, though, you can always add to it as your business grows.

One thing you should include, though, is market research. The last thing you want to do is go through all of these "how to start a day care business" steps, only to find that there isn't a market for one or that there are already too many day cares in that area to make it a viable business.

Your business plan should also include a budget. The costs associated with opening and running your day care center can never be accurately totaled, but nailing down a budget will give you some parameters to work within (and some peace of mind).

In your budget plan, don’t forget to factor in:

Your startup costs, including day care equipment, food, toys and educational tools, wages, insurance and licensing.

How much tuition you’ll charge.

Your predicted revenue over the next two to three years.

When you’ll break even.

Also know that day care centers can claim certain tax deductions, which can ease your annual financial burden.

You'll also want to include a marketing plan. If you’re seriously researching how to start a day care business, it’s likely that you’ve already been caring for children in your area for some time and have built up a network of local parents. That’s one valuable method of attracting customers (aka word of mouth) covered. Still, implementing even a basic marketing plan can help define and legitimize your business — and if you need to implement a waitlist as a result, that’s great too!

Your marketing efforts can be relatively simple and low-cost. You can start by creating a Facebook page for your day care center and building a business website, making sure to include your contact information and a little bit about your business. If possible, plan to include pictures of your facilities and testimonials from happy customers.

If you’re a fan of social media, it also can’t hurt to create an Instagram, LinkedIn and/or Twitter account to keep both current and prospective customers updated on your business. Just be sure that once you do, you receive permission from parents or guardians before posting pictures of their children, of course.

Analog marketing techniques would work well here, too. If possible, consider distributing flyers or brochures to nearby libraries, schools, places of worship or any other local gathering spots.

3. Obtain the necessary certification and licenses

You may not need a master’s degree in education to become a day care teacher, but each state does require some combination of licensing and certification when it comes to how to start a day care. Visit your state’s Division of Child Care Services (or its equivalent) to find out the training, experience and credentials you need to legally operate a day care facility.

In New York State, for instance, the head of the day care facility must have one of the following:

An associate degree in early childhood education or an equivalent.

A CDA credential (child development associate) and at least two years of experience caring for children.

A high school diploma and at least three years’ experience caring for children.

You'll probably find that there are some other requirements that usually come up when you're looking up how to start a day care business at home or in another facility. Your state may require that you and any staff you hire are CPR-certified, for one. Another point of safety to remember is that you should be trained in at least basic first aid for children. You and your staff might also need to be fingerprinted and undergo background checks before being cleared for work.

Even if your state doesn’t require that you obtain a license, you should consider doing so, as your licensing course will go over all the boxes you need to check to operate your day care in your state — including health and safety regulations, proper food preparation and the required child-to-adult ratio.

4. Find a (safe) day care facility

In certain states, in order to obtain the proper licenses or registration to start your day care business, you’ll first need to show that your day care facility meets your state’s health and safety requirements.

So whether you choose to open your day care facility in your own home, or buy or lease a new property, you’ll need to make sure your facility meets zoning, fire, and health and safety laws. When it comes to how to start a day care at home, you may need to make alterations to your house to comply with day care requirements in your state, so be sure to factor them into your budget.

5. Get insured

Another requirement for obtaining your license? Getting insured. In certain states, you’ll need to be licensed by the Department of Health and Human Services to run your day care center, but to do so, you’ll first need liability insurance.

The exception is home-based day care centers, which don’t need insurance to be licensed by the DHHS. Still, those opening day care centers at home should seriously consider obtaining small business insurance. You always run the risk of a lawsuit when you’re running your own business, but that risk increases when caring for other people’s children — so protect yourself whenever possible.

There are several types of insurance that cover day care businesses — general liability insurance, workers’ compensation, property insurance, abuse and molestation insurance and others — so contact an insurance broker to help you decide which coverage is best for your business.

6. Get a business checking account and credit card

Get a business credit card and a business checking account early on to help you keep your business expenses separate from your personal expenses when first starting a day care.

It’s crucial to separate your personal and business expenses for many reasons, not the least of which is to reduce your (or your accountant’s) burden come tax season. It’s simply the most professional way to conduct your business, too. Set yourself up for success now by signing up for a business credit card and opening a business bank account, and be diligent about using both solely for your day care’s finances.

Having a business credit card can help you increase your business credit score, too. By paying the card off on time — or early if you can — you can boost your score, which will help you late on when you need a loan or other financing for your day care business.

7. Get financing

Most entrepreneurs bootstrap their businesses at the beginning, as it’s difficult for brand-new businesses with limited credit history to secure a business loan, either from a bank or from an alternative lender.

Your financing options aren’t limited to your own purse strings (or your friends’ and family’s). Here are a few other ways to get funding and loans for a child care business:

SBA microloan

Although most SBA loans are available only to businesses with a few years of experience under their belts, SBA microloans are actually designed to help startups get off the ground. They’re especially accessible to women, veterans, minorities and business owners in low-income areas. And unlike most other SBA loan programs, SBA microloans are disbursed by nonprofit lenders, rather than banks.

As the name suggests, SBA microloans tend to be on the smaller side, with amounts capped at $50,000, but they may be as low as $500. (For more context, the average microloan amount was $14,000 in 2017.) And because microloans are designed for new businesses, business owners with average or even challenged credit may still be accepted, as long as other aspects of their SBA loan applications are in good shape.

SBA community advantage

These loans from the SBA have all of the advantages that come with SBA microloans but the community advantage loans are specifically for businesses that are serving traditionally underserved communities. These loans are generally for a higher dollar amount than the microloans and can go a bit further for your business.

Government grants

You might be able to obtain financial assistance to start and run your day care through state or federal funding programs for early childhood education providers. For instance, you can contact your state’s Child Care and Development Fund Plan to look into startup funds, or your state’s school meal contacts to seek funding for your day care center’s meals.

Business credit card

It’s important to use a business credit card to keep your day care’s expenses separate from your own. Of course, using a credit card is also the most convenient way to pay for your daily expenses. And since credit limits for business credit cards tend to exceed those of consumer cards, you can spend more liberally without worrying about maxing out your card.

Another bonus? Using your business card responsibly (by which we mostly mean paying off your balance in full and on time, every month) can help you build business credit. And with a healthy business credit score, you’re in a better position to secure business loans with great terms down the line.

If you opt for a card with a long 0% intro APR period, you can essentially use that introductory grace period as an interest-free loan. Look into the American Express Blue Business Plus card, which, at 12 months, carries one of the longest interest-free introductory periods in a business credit card right now.

After your 12 interest-free months are up, though, a variable APR kicks in at a rate depending on your creditworthiness and the market. Check the issuer's terms and conditions for the latest APR information.

8. Hire staff

You may be planning on running your day care facility solo, but depending on your state and the number of children you’re looking after, that might not be an option — every state sets a required ratio of staff to children to ensure that every child receives adequate care. They also dictate the maximum number of children permitted in a group.

Adult-to-child ratios and class sizes depend on the age of the children, but they might also depend on the size of the day care facility, or face further restrictions based on municipality. For a day care center in New York state, for example, the state requires one adult for six children under school age. However, in New York City, there must be two teachers or one teacher and one assistant to every six children aged 2 to 3, with a maximum of 12 students allowed in a single group. Your own children may or may not be included in that count, too.

So, while hiring really depends upon your state’s requirements, it makes sense to leave room for hiring staff in your business budget. That way, you’ll be prepared for growth, without worrying about your operation shutting down because you’re not properly prepared for it. (Just keep in mind that any staff you hire needs to be appropriately licensed or trained for it, and potentially undergo a background check.)

9. Write your day care contract and policies

To further legally protect your business, it’s crucial that you draw up a contract, write out your day care policies, and require that potential clients (or, more likely, the parents of potential clients) review and sign both documents before accepting their patronage.

If you're just starting to read up on how to start a day care business, you might not be clear on the distinction between these two documents. To clarify, your contract is the document stating that you’ll provide child care, be compensated for providing care according to the payment terms you specify, and have the right to terminate providing care.

Your policies, on the other hand, provide parents with important logistical information regarding how you’ll run your day care center. There, you can outline protocol regarding vacation, illness, inclement weather, drop-off and pickup times, curriculum, field trips, and anything else you believe is important for your clients to understand and agree to about your day care center.

In this article we go through, step-by-step, all the different sections you need in your daycare business plan. Use this template to create a complete, clear and solid business plan that get you funded.

1. Executive Summary

The executive summary of a business plan gives a sneak peek of the information about your business plan to lenders and/or investors.

If the information you provide here is not concise, informative, and scannable, potential lenders and investors will lose interest.

Though the executive summary is the first and the most important section, it should normally be the last section you write because it’s the summary of the different sections included in your business plan.

Why do you need a business plan for your daycare?

The purpose of a business plan is to secure funding through one of the following channels:

  • Obtain bank financing or secure a loan from other lenders (such as a SBA loan )
  • Obtain private investments from investment funds, angel investors, etc.
  • Obtain a public or private grant

How to write an executive summary for your daycare?

The executive summary of your daycare business plan should include the following important information:

Business Overview

Introduce your company (its name, its mission) and the history behind it: why did you decide to create a daycare in your area today? Why you? 

Also, that’s where you should expand on the business: where will the daycare facility be located? How old are the children you target? How many children will you be able to take care of? What are the amenities (classrooms, playground, cafeteria, etc.)? 

Market Overview

Provide here a deep market analysis that backs your decision to open a daycare business in your area today. Why would your business succeed given current market conditions? 

For example, the market analysis should include information like: what are your competitors in the area? What are their characteristics, strengths and weaknesses? Who are your target audience (parents and children)? Is that in line with the demographics in your area?

Management & People

Who is the management team? What is your/their experience in the daycare industry?

Financial plan

What is your expected revenue and profitability for the next 5 years? When do you expect to break-even? Simply include here a chart of your key financials (e.g. Revenue, Gross Profit, Net profit )

Funding Ask

What loan/investment/grant are you seeking? How much do you need? How do you intend to spend the money?

2. Daycare Business Overview

The business overview section of the daycare business plan summarizes the basics of your facility, including the background information, business model, services, target audience, and legal structure. 

a) History of the Project

When you launch a daycare business, you want it to grow and even become the best in the region if possible. One small element that can catalyze your company’s growth is its history. You don’t have to exaggerate the information here, but try to include useful details that will make your daycare business stand out. 

For a business as sensitive as daycare, trust overrides anything else. So, make sure to include in your business plan your experience and passion for children to demonstrate to investors you are qualified and the right person to successfully run a new daycare facility.

Also, flesh out the history behind the project: why are you starting a daycare now? For example, you may have noticed a lack of child care services for toddlers and infants in the city.

b) Business Model

Next up is the business model. This is the revenue-generating plan that identifies how your business operates. A daycare business model should be succinct and address specific things about the business. 

For instance, is it a commercial daycare facility or a family daycare? Is it an independent facility or a partnership? And should you opt for a daycare franchise? 

Keep in mind that daycare facilities vary by the target age group. So, you can opt for a childcare center, a family daycare, a kindergarten/pre-school, or a nursery school.

c) Daycare Services

It takes a lot to raise a child. So, be clear on the services you want to offer in your newly founded daycare facility. The quality of services will make your business more attractive. But an even bigger factor will be the type of services you provide at the facility.

Indeed, a lot happens inside a daycare facility, from hands-on learning (classrooms, private tuition) to field trips, games and community events. 

d) Pricing Strategy

Age is a key factor when setting the prices of your daycare services. On average, parents across the US pay about $9,400 per year on child care per child. Of course, the actual figure will vary based on a number of factors, but this is a great starting point when getting into the industry. 

An accurate pricing strategy can help you outshine your biggest competitors. However, don’t forget that setting cheaper prices isn’t necessarily the best strategy, just in the same way overpriced services may turn off potential clients. 

e) Target Audience

Daycare centers are so named because they are about a child’s well-being. However, the final decision rests with the parents, who must buy into your idea and long-term vision for the facility. 

For this type of business, you have a definite target market. And all you have to do is ensure your facility is closer to a busy neighborhood with many children. Find out if the families truly need child care services, why they need these services and the children’s average age. 

Finally, make sure your services are aligned with your target audience. For example, you wouldn’t necessarily succeed with a daycare operating 9am – 4pm in an area where parents typically work long hours in the city and need time to commute back to their neighborhood to pick up their kids.

Same goes for pricing: if you offer high-quality expensive daycare services, make sure your daycare is either located close to offices or in an area where affluent parents work or live.

f) Legal Structure

Finally, your business overview section should specify what type of business structure you opt for. Is this a corporation or a partnership (LLC)? Who are the investors? How much equity percentage do they own? Is there a Board of Directors? If so, whom? Do they have experience in the industry?

In the market overview section of your business plan, you must cover 2 important areas:

  • Market trends : how big is the daycare industry in your area? How fast is the market growing? What are the trends fuelling this growth (or decline)?
  • Competition analysis : how many competitors are there? How do they compare vs. your business? How can you differentiate yourself from them?

It’s always helpful to base your business decisions on the latest trends in the US market. For instance, the US daycare market had a value of approximately $54.3 billion in 2019 . And it is projected to grow at an annual rate of 3.9% from 2020 through 2027. 

According to reports, the high number of parents occupying full-time and part-time jobs is a major driving force behind the increasing demand for daycare services. No matter how you look at it, these statistics make the daycare business even more lucrative, provided you get all the basics right through your business plan.

How big is the daycare industry in your region?

After the US, assess the size of the daycare market in your city or area. Focus on the zone where you plan to offer daycare services.

Naturally, you might not be able to get the data for your specific city or region. Instead, you can estimate the size of your market, for more information on how to do it, read our article on how to estimate TAM, SAM and SOM for your startup . To give you an example, let’s assume you plan to operate in an area where there are already 10 competitors:

As we know the US daycare industry is worth $54 billion today, and there are about 230,000 child care centers , therefore the average annual turnover per child care center is around $235,000.

Now, we can safely assume that the daycare industry is worth $23 million in your area (10 centers).

How fast is the daycare industry growing in your region?

Growth is an important metric for assessing the status of the daycare industry in your region. 

Here if you don’t find information online or via your research, you can calculate growth using the total number of competitors in your area. 

For example, assuming there were 8 daycare competitors in the region in 2018, and 10 in 2022, the annual growth rate is 6% per year.

b) Daycare Competitor Analysis

At the very least, your competitor analysis should answer all the questions below:

  • How many daycare businesses are the area where you plan to open yours?
  • What type of daycare businesses are there: home-based vs. center-based home care, early care vs. early education & daycare, etc.
  • What age range do they specialise in?
  • What services do your competitors offer?
  • What amenities do your competitors have (playground, classrooms, etc.)
  • What’s their average price (daily rate / monthly rate)?
  • What is the child / staff ratio of your competitors?

4. Sales & Marketing Strategy

For some existing daycare facilities, marketing isn’t the most important aspect of running the business. But you’ll probably have to implement a few marketing strategies at the beginning to attract the first families , especially if you’re starting a new daycare facility.

a) Daycare Market Channels

A daycare business doesn’t have diverse marketing channels like retail stores or other businesses. So, this may limit your options when it comes to new marketing channels. 

Apart from word of mouth, other marketing channels include;

  • Social media

b) What are Your Unique Selling Points (USPs)? 

Daycare facilities offer pretty much the same services, and it’s not easy to stand out from the competition. However, a few factors can be useful when evaluating your opportunities in a competitive market, including:

  • Target age group : You may cover a unique age group as opposed to your competitors
  • Opening hours : you may offer longer opening hours to accommodate for different parents’ jobs and availabilities
  • Price : Your services may be cheaper than your competitors
  • Quality & amenities : Quality services and extra amenities (outdoor playground, etc.) will make your daycare facility more attractive vs. competitors
  • Services : Your services may go beyond the standard hands-on learning and kid games 

5. Management & People

The 5th section of your daycare business plan should be about people. It should include 2 main elements:

  • The management team and their experience / track record
  • The organizational structure: what are the different teams and who reports to whom?

Here you should list all the management roles in your company.

Of course, the amount of details you need to include here varies depending on the size of your company. For example, a small daycare business run by 1 or 2 persons doesn’t need the same level of detail vs. a large center with 50 children or more.

If you plan on running your business independently, you may write a short paragraph explaining who are the co-founders and/or senior managers (if there are any in addition to yourself). It’s important to highlight their experience in the industry and previous relevant professional experiences.

b) Organizational structure

No matter how many leadership roles there are, you should now explain how you intend to run the company from a management standpoint.

What are the different teams (management, childcare staff, cooking staff, human resources, finance, etc.)?

Note that you should include these details even if you haven’t hired anyone yet. It will show lenders and investors that you have a solid hiring and management plan to run the business successfully.

A great addition here is to add an organizational chart that list all the roles, from Directors to managers, key supervisory roles and employees. Make sure to highlight with reporting lines who manages/supervises whom.

6. Financial Plan

The financial plan is perhaps, with the executive summary, the most important section of any business plan.

Indeed, a solid financial plan tells lenders that your business is viable and can repay the loan you need from them. If you’re looking to raise equity from private investors, a solid financial plan will prove them your daycare is an attractive investment.

There should be 3 sections to your financial plan section:

  • Your historical financials (only if you already operate the business and have financial accounts to show)
  • The startup costs of your project (if you plan to start a new daycare facility, or add capacity to an existing daycare center, renovate your facilities, etc.)
  • The 5-year financial projections

a) Historical Financials (optional)

In the scenario where you already have some historical financials (a few quarters or a few years), include them. A summary of your financial statements in the form of charts e.g. revenue, gross profit and net profit is enough, save the rest for the appendix.

If you don’t have any, don’t worry, most new businesses don’t have any historical financials and that’s ok. If so, jump to Startup Costs instead.

b) Startup Costs

Before we expand on 5-year financial projections in the following section, it’s always best practice to start with listing the startup costs of your project.

For a daycare, startup costs are all the expenses you incur before you open the space to your customers. These expenses typically include: renovation costs, equipment and furniture, etc.

The startup costs for opening a child care center depend on various factors such as the location and size of your daycare facilities, the capacity (the number of children you plan to have), the quality of the amenities, etc. 

We’ve identified that it costs anywhere between $130,000 to $490,000 to start a daycare business with 50 children . See below the cost estimates.

Note that these costs are for illustrative purposes and depend on several factors which might not fully apply to you. Let’s first start below with startup costs. 

c) Financial Projections

In addition to startup costs, you will now need to build a solid daycare financial model over 5 years.

Your financial projections should be built using a spreadsheet (e.g. Excel or Google Sheets) and presented in the form of tables and charts in your business plan.

As usual, keep it concise here and save details (for example detailed financial statements, financial metrics, key assumptions used for the projections) for the appendix instead.

Your financial projections should answer at least the following questions:

  • How much revenue do you expect to generate over the next 5 years?
  • When do you expect to break even?
  • How much cash will you burn until you get there?
  • What’s the impact of a change in pricing (say 10%) on your margins?
  • What is your average customer acquisition cost?

You should include here your 3 financial statements (income statement, balance sheet and cash flow statement). This means you must forecast:

  • The number of children over time ;
  • Your expected revenue ;
  • Operating costs to run the business ;
  • Any other cash flow items (e.g. capex, debt repayment, etc.).

When projecting your revenue, make sure to sensitize pricing and the number of members as a small change in these assumptions will have a big impact on your revenues.

7. Funding Ask

This is the last section of the business plan of your daycare center. Now that we have explained what type of daycare services your company would offer, at what price, your marketing strategy, management and people, this section must now answer the following questions:

  • How much funding do you need?
  • What financial instrument(s) do you need: is this equity or debt, or even a free-money public grant?
  • How long will this funding last?
  • Where else does the money come from? If you apply for a SBA loan for example, where does the other part of the investment come from (your own capital, private investors?)

Use of Funds

Any business plan should include a clear use of funds section. This is where you explain how the money will be spent.

Will you spend most of the loan / investment to buy the real estate and do the renovations? Or will it cover the cost of the salaries of your childcare staff and other employees the first few months?

Those are very important questions you should be able to answer in the blink of an eye. Don’t worry, this should come straight from your financial projections. If you’ve built solid projections like in our daycare financial model template , you won’t have any issues answering these questions.

Then, determine how you will measure the success of each milestone by listing out the key performance indicators and metrics needed to see progress. These will be the numbers you monitor on a regular basis to evaluate the trajectory of your business. The structure of your staff and teams sets your daycare up for success. A typical financial plan will have monthly sales and revenue forecasts for the first 12 months, and then annual projections for the remaining three to five years. Break your sales forecast down into several rows, focusing on just high-level buckets at this point.

You can't market to everyone, so you must identify your target market. This will help you focus your marketing efforts and ensure that you're reaching the right people. 

Below is a daycare business plan example that shows how your business description should be:

“ABC Daycare is a small, home-based daycare located in San Francisco, CA. We cater to children aged 0-12 years old and offer full-time, part-time, and drop-in care. 
Our curriculum is based on the Reggio Emilia approach, emphasizing hands-on learning and collaboration. Activities and programs include arts and crafts, music, and outdoor play. 
Our facility can accommodate up to 12 children at a time. We have a staff of four employees who are all CPR and First Aid certified. 
Our target market is working parents in the city who need quality child care but can't afford the rates of larger daycare centers.  We've created an affordable subscription-based pricing model for our target market to fulfill the demand. We generate revenue through monthly subscriptions and have low operating costs due to our small size. 
Our suppliers are local businesses that provide us with food, toys, and other supplies.” 

Do a Thorough Market Analysis

After writing a compelling description of your business, you need to do a thorough marketing analysis. This analysis will help you determine your target market, what type of advertising and promotion will work best, and how to price your services. 

You should also research the competition and see what they are doing right and wrong. This information will be invaluable as you create your daycare business plan.

Keep these things in mind when doing a market analysis:

The Size of Your Market

This is determined by the number of potential customers in your area who need or want your services. 

For example, if you live in a small town with only a few thousand people, there may not be enough demand to support a large daycare facility. 

On the other hand, if you live in a city with hundreds of thousands of people, there may be room for multiple daycare facilities. 

Your target market is the segment of the population that is most likely to use your services. This includes factors like age, income, education, and location. 

After you've identified your target market, you need to show how you plan on fulfilling the demand. This is where your business model comes in. 

Your business model is a detailed description of how your daycare will operate daily. It should include: 

  • How do you plan on acquiring customers? 
  • What are your pricing strategies? 
  • How will you generate revenue? 
  • What are your operating costs? 
  • Who are your suppliers? 

Your business model should be detailed and easy to understand. It should also be realistic and achievable.

“The daycare will be open Monday through Friday from six in the morning to six at night. We will offer care for children ages six weeks to twelve years old. 
Our rates will be $50 per week for one child and $40 per week for each additional child from the same family. We will offer a discount of $20 per week for families who enroll their children for an entire year. 
We will generate revenue by charging weekly rates for our services. Our operating costs will include rent, utilities, supplies, and salaries for our employees. Also, we will acquire customers through online advertising and word-of-mouth.” 

As you can see, a business model is a detailed description of how your business will operate. It's essential to have one in place before promoting and selling your services. 

One thing you should not forget to include in your daycare business plan is the location of your business and your rental agreement. If you are renting a space, including the terms of your agreement and how long you have the space. If you are purchasing a property, include information on the property, such as square footage and any special features that will help your business stand out. 

This daycare business plan example shows you how to include this vital information: 

“The daycare will be located at 123 Main Street in a commercial space currently leased by the owner. The lease agreement is for three years with an option to renew for an additional three years. The monthly rent is $2000, and the security deposit is $3000. 
The daycare will have exclusive use of the main floor, including a large open play area, a small kitchen, two bathrooms, and four classrooms. The daycare will also have access to the outdoor playground.
80% of our space will be used for childcare, with the other 20% used for our administrative offices and staff lounge. 
We have chosen this location because it is close to several residential neighborhoods and has easy access to public transportation. The space is also large enough to accommodate our future growth.”

Daycare Marketing Plan

Most daycare business plan templates will include a section for your marketing plan. Most people overlook the marketing aspect of their business, but it is one of the most important pieces of your puzzle.

Daycare Financial Plan

Your business plan should include a financial plan section. This is where you'll lay out how much money you need to start or grow your business. Be specific and include dollar amounts. If you're seeking a loan, including information on how much you're requesting and how you'll use the funds.

You should also include a detailed budget in your business plan. Your budget should include all of your projected income and expenses for at least the first year of operation. Creating a budget will help you get a clear picture of what it will cost to start and operate your business.

This section should include projected costs for:

  • Rent or mortgage payments
  • Advertising and marketing
  • Operating expenses such as utilities, supplies, and more. 

Startup costs are another vital item to include in your business plan. This is the money you need to purchase equipment, furniture, or any other items to get your business up and running.

If you plan to secure a loan, your lender will want to see a detailed business plan with information on how you plan to use the loan funds. Ensure you include this information in your business plan to increase your chances of securing funding.

If you're seeking funding from investors, you'll need to include information on how they will be compensated. This is typically done through equity, a percentage ownership stake in your business.

How much does it cost to start a daycare? 

It costs between $10,000 and $50,000 to start an in-home daycare business according to Bizfluent . 

It costs between $59,000 and $3 million dollars to start a daycare facility according to Bizfluent . 

So obviously this is a huge range in startup costs.  The main thing that will determine your startup costs is your daycare facility.  Depending on how large your daycare is, whether you are buying, building, or leasing the space, and how much renovation needs to be done, your startup costs can vary drastically.  

Some tips to help you estimate a cost of a daycare facility:

  • A daycare facility should have 35 square feet of open floor space indoors per child. 
  • So if you wanted a facility that could care for 100 children you would need 3,500 square feet of indoor space for children, plus additional space for offices, kitchen, bathrooms, etc.  Let’s assume that you would need at least 5,000 square feet of space for a daycare facility that served 100 children.  
  • A daycare center would cost at least $295 per square foot to construct in the U.S. based on data from Levelset . 
  • Constructing a new 5,000 square foot daycare center would likely cost at least $1,475,000 based on $295 per square foot.  
  • Now you might not be constructing new, rather you might rent an existing facility which could require renovations.  You will need to get a specific quote for the specific renovations that you need for your space. 

How much revenue can a daycare business make?

A daycare facility can generate $17,680 in revenue per year per child according to Zippia .

  • Exit strategy : This is the plan for how you will eventually sell or otherwise exit the business in case you decide to retire or move on to other projects.

The executive summary is the most crucial section of your business plan because it gives investors and lenders a quick overview of your company and its prospects. Be sure to include all of the key elements listed above, and keep it under two pages in length.

Gov. J.B. Pritzker’s ‘more austere’ $52.7 billion budget plan turns to big business, sportsbooks for tax increases

There will undoubtedly be sticking points for some of the governor’s priorities, including about $182 million in funding for newly arriving migrants and more than $600 million in health care costs for the undocumented..

SPRINGFIELD — Gov. J.B. Pritzker on Wednesday unveiled a $52.7 billion budget that includes the blueprint for a pension overhaul, funding to care for newly arriving migrants and a new set of costs for businesses.

It’s also a plan that Republicans say prioritizes the needs of migrants and the undocumented over Illinois residents.

While calling the state’s financial future “bright,” the sixth spending plan of the Democratic governor’s tenure leans on large corporations and lucrative casino sportsbooks to churn out more than $800 million in revenue for the state.

Those proposals were not mentioned in the governor’s 52-minute joint State of the State and budget address before the Illinois General Assembly.

Instead, Pritzker focused on some of his key priorities, including early education, social services and health care. He defended the state’s response to the migrant crisis — while also railing against former President Donald Trump — and the state’s Republican congressional delegation — for rejecting an immigration bill that would have revamped the country’s border policies and helped Illinois.

“Listen, maybe some of you think we should just say, ‘This is not our problem,’ and that we should let the migrant families starve or freeze to death. But that’s not what decent Midwesterners do. That’s not what leaders do,” Pritzker said. “We didn’t ask for this manufactured crisis. But we must deal with it all the same.”

Pritzker, who also serves as a key surrogate for President Joe Biden’s reelection campaign, also acknowledged “our immigration system has been broken for a long time.”

“No doubt, the current migrant crisis is a problem of the federal government’s making, and I mean both political parties,” the governor said.

Some Republicans groaned when Pritzker called Biden a “very good president who has rescued the economy and protected freedom.”

New revenue

The governor is projecting a 1.5% increase in state revenue for the new fiscal year, which begins July 1, for a haul of nearly $53 billion. That’s largely due to the $800 million plus in tax increases mostly targeting large businesses.

Pritkzer’s plan extends a limit on the amount of operating losses corporations can write off on their income taxes. A $100,000 cap that was slated to expire at the end of the year would be extended three years, under the plan from Pritzker's team, which aims to soften that blow on businesses by upping that cap to $500,000. The maneuver would generate another $526 million for the state, the governor's office estimates.

The governor’s office pushed back on characterizing it as a new tax, arguing the continuation of the cap on corporate tax deductions will help the state keep gaining revenue.

The governor also wants to more than double the state tax on sports betting revenue collected by sportsbooks from 15% to 35%, a proposal that’s sure to draw pushback from a rapidly expanding industry that raked in more than $1 billion in 2023. The governor’s team predicts such a hike could pump an additional $200 million into state coffers.

And he wants to lower the tax discount retailers receive for collecting sales taxes, a measure his office says would net the state an additional $101 million. In his speech, Pritzker also said he wants to permanently eliminate the 1% grocery tax. But the Illinois Municipal League already opposes that initiative — saying the tax goes entirely to local governments across the state and would cost them $325 million a year.

“This loss in revenue is entirely borne by local governments and is clearly punitive toward municipalities,” Brad Cole, CEO of the Illinois Municipal League said in a statement. “Does anybody think this is a good time to cut the city of Chicago’s revenues by tens of millions of dollars a year?”

The plan also includes transferring mass transit costs from sales tax to the state’s Road Fund, to take in an additional $175 million.

The Illinois Chamber of Commerce panned two of the revenue proposals, calling the cap “nothing more than forced borrowing of funds from Illinois businesses to finance government,” and the collection discount reduction “a stealth tax on our retail sector.”

Pension overhaul

Pritzker’s budget team is also aiming to hasten the state’s pension funding ramp with an eye toward landing another credit upgrade from Wall Street ratings agencies.

Since 1994, the state has been on a slog toward filling the gap in the grossly underfunded system to 90% by 2045. Pritzker’s team is adjusting that goal to reach 100% funding by 2048 — closer to pension goals set by many other states.

The governor’s team says they can make that happen in part with savings from paying off two other major bonds issued by the state over the next decade. Pritzker will need legislative approval for the pension proposal.

The $52.7 billion in spending amounts to an increase of about 2% compared to last year’s plan. Officials on Wednesday morning called the plan a “more austere budget than in recent years” — which also maintains a promise of fiscal responsibility. Republicans noted spending plans in Illinois, led by Democrats, keep growing.

There will undoubtedly be sticking points for some of the governor’s priorities, including funding for migrants and the health care costs for the undocumented.

The governor’s budget includes $181.7 million to continue to care for newly arriving migrants, and $629 million to continue providing health care benefits to undocumented people aged 42 and up who would otherwise qualify for Medicaid.

Those issues drew swift rebukes from Republicans.

Illinois House Republican Leader Tony McCombie, R-Savanna, said the proposal places a burden on small businesses and is sorely lacking in reforms.

“We can’t tax our way into success. We need to have actual structural and tax reform, and we don’t see that,” McCombie said. The Republican leader also questioned whether the sports betting tax hike will be paying for “pork projects” for Democrats.

Illinois Senate Minority Leader John Curran, R-Downers Grove, argued Pritzker "just proposed raising taxes on every Illinois family struggling to make ends meet to fund the non-citizen welfare state he created.

"We have made it clear that the citizens of this state are our priority, while today, the Governor made it clear they’re his piggy bank. Our focus will remain on providing meaningful financial relief to the people of Illinois," Curran said in a statement.

Education dollars

Education funding would include $350 million towards the state’s school funding formula — although advocates have been warning they would put up a fight for more than that. The total K-12 funding under the governor’s proposal totals $10.8 billion in general funds.

The proposal also includes $400 million for year two of Pritzker’s Smart Start program, which aims to add 5,000 new slots and expand access to early childhood education. It also includes $13 million to launch a Department of Early Childhood.

The governor proposed a much more modest increase in funding for state grants for low-income college students than in previous years. Last year he secured a $100 million increase for the Monetary Award Program, or MAP grant , and this year advocates had called on him to request $50 million more. But he's asked lawmakers to approve just $10 million more for the state financial aid program, which would bring total funding for MAP grants to $711 million.

The proposed budget creates a $12 million child tax credit for lower-income families with children under age 3, for a credit of about 20% of the taxpayer’s state earned income.

The spending plan sets aside $175 million for the state’s “rainy day” fund, an emergency piggy bank that has gone from nearly insolvent in 2017 to $2.3 billion by next year, if passed by the General Assembly.

The budget also includes $10 million in federal funds to erase $1 billion in medical debt for Illinois residents — the first in a three-year plan.

Budget negotiations are expected to continue through the end of May, the deadline for lawmakers to pass a spending plan.

A “Times” investigation has found that doctors are increasingly performing unnecessary medical procedures that generate huge profits while often harming patients.

Today, my colleague Katie Thomas — on the forces driving this emerging and troubling trend in American health care and the story of one family caught in the middle of it. It’s Monday, February 19.

So Katie, tell me about this investigation.

So I am a health care reporter who writes about the kind of intersection of health care and money. And I was working with two other colleagues, Sarah Kliff and Jessica Silver-Greenberg. And together, the three of us had long been interested in, are the medical procedures and the tests and other things that we get when we go to the doctor or into a hospital — are they always necessary?

But what we were really interested in exploring was not just are these procedures and are these tests, et cetera — are they necessary, but in some situations, could they actually be harmful to patients? And so that’s what we decided to try and take a look at. And so we had gotten started in our reporting when we got a tip. And it was from a mom in Boise, Idaho. And her name was Lauren Lavelle.

Hi, how are you?

And my colleague Jessica Silver-Greenberg and I went to her house to meet with her.

And where does her story start?

I am a mom of two. I live in Boise. My daughter, June, is four, and I have a 17-month-old, Flora.

Her story starts when Lauren gets pregnant with her daughter, June.

So by the time we got pregnant with June, November of 2018, about eight months after we had the miscarriage, I think I was just more hesitant and nervous than anything.

Lauren and her husband had trouble conceiving, and so they were so happy when they learned that they were going to have June. And like most first-time parents, they were also a little bit nervous.

But being type-A and super prepared, I did all my homework. We hired a doula. I wanted an epidural. Having a natural childbirth absolutely was not for me.

And Lauren is very organized. She’s always on top of everything, and she makes all sorts of plans. And she gets a lot of different providers lined up ahead of time —

I didn’t know anything about breastfeeding, like zero things.

— including one that she has hired to help her with breastfeeding.

Where did you find out about her?

So I asked our doula for a list of recommendations, and she gave me a very short list. At the time, there were very few lactation consultants in the Valley. And Melanie was one of them.

She ended up deciding to work with Melanie Henstrom, who was a local lactation consultant in Boise.

She sold this package at the time. I don’t know if she still did, but it was like prenatal visit breastfeeding class. And then, she’ll come to the hospital and help you latch, and then she’ll come to the house a couple of times after. And I thought, well, this sounds perfect. Great. You know, I’m covered there.

So one week after her due date, she gives birth. And it was a difficult labor. It took 24 hours. Lauren was completely exhausted. But once June arrived, the family was very, very excited to have her.

And I remember June coming out and that surreal feeling have when you see your first baby for the first time, like oh, my God, there’s a baby in the room.

And June was a healthy baby, but she was having trouble breastfeeding.

She would not latch. Like, she wouldn’t even attempt. She would scream. It was the only time she ever cried — if you tried to make her to breastfeed.

And so as her pediatrician was making the rounds, they noticed that June was having trouble and said that June’s tongue is really tight.

We can clip it if you’d like.

And that they could clip it.

What does that mean exactly, Katie — clipping her tongue?

What it means is that there’s a small percentage of babies whose tongue is very tightly tethered to the bottom of their mouth. And for a very small percentage of babies, their tongue is almost tied so tightly down that they can’t nurse well.

So it makes breastfeeding very difficult if a baby has a tongue like this.

Exactly. If you bottle-feed your baby, the baby can basically adjust and make do. But if you want to breastfeed, some babies have trouble, basically, latching on to their mother when they don’t have that tongue motion. And so some version of clipping these tongue ties has been done for centuries. Midwives have been doing it. Pediatricians do it.

And traditionally, what it’s been is a very quick snip right underneath the tongue just to loosen up the tongue. And traditionally, that procedure is extremely straightforward. There’s little to no follow-up care. And basically, the baby naturally heals as it learns to breastfeed.

And so we said, OK. They explained that it was completely painless. They’d do it with scissors. She wouldn’t even feel it. And all of that was true. They clipped it. I don’t even think she woke up.

But in June’s case, it didn’t seem to help much, and she and Lauren were still having problems breastfeeding afterwards. So while she’s still in the hospital, she calls up the lactation consultant that she had hired — Melanie Henstrom — just to let her know what was going on. And from talking to her on the phone, Melanie said that the situation was actually much worse than Lauren had thought and that Lauren’s baby needed another tongue-tie procedure — a deeper cut under the tongue.

How did she make this diagnosis, Katie? Was it over the phone? How did she know this?

Yes, Lauren told us that it was from a phone conversation. And in addition to that, she also warned her that, basically, Lauren and her husband should really take this seriously and consider getting it done, because if she doesn’t get it fixed, it could lead to a whole host of problems beyond just problems breastfeeding.

She’ll have scoliosis, and she’ll suffer from migraines, and she’ll never eat, and she’ll have a speech impediment, and she won’t sleep — I mean, just, like, the long list of things over the phone.

And Lauren starts panicking.

I mean, first of all, I felt — I’ve never felt more terrible in my life than that first day or so after giving birth. Like, the comedown from the hormones, the drugs — all of it — the sleep deprivation. And then, here was this baby we’d wanted, we were told we probably would never have after one miscarriage. And she’s so perfect, like, the most beautiful baby I’d ever seen. And you think that she has some deformity that’s going to ruin her.

But Melanie says it’s OK. She has a solution. And she tells Lauren that there’s a dentist in town who can handle cases that are as severe as June’s.

A dentist? Why a dentist?

Well, there’s a procedure that’s done in a dentist’s office that’s a laser surgery. And dentists use this high-powered laser machine that can quickly cut the flesh that connects the lips and the cheeks to the gums. So according to Lauren, Melanie tells her that by chance, this dentist has an opening, because she said a family coming in from Oregon had just canceled their Saturday appointment.

So I thought, OK, wow, people are coming in from Oregon to see him. So we talked about it. We both felt unsure. But we said, well, let’s at least take the appointment, and then we can at least meet with the dentist, and also, someone can look at her mouth and assess.

And so Lauren agrees to go in and meet the dentist.

Like, I think some people, when they hear this story, think, like, why would you believe that? It just sounds so scammy. But to me, there is a lot of things that you hear in the hospital that sound insane. Like, it’s no different than someone saying, like, your baby’s orange because their bilirubin levels are too high, so we got to go put them under these lights. Like, that sounds insane. That sounds more insane than, your baby’s having a hard time eating because their tongue is too tight and it needs to be cut. Like, that seems rational, actually.

And all of this seemed really weird to Lauren at the time. But in the context of the hospital and having a baby, lots of things about health care are weird.

So one day after they got back home from the hospital, Lauren and her husband pack up the car and go to the office early in the morning.

You know, I was wearing my hospital diaper and an ice pack, took the elevator up to his office, and —

And what happens?

So Melanie greets them at the door. They sign some paperwork, and pretty soon, the dentist, Dr. Samuel Zink, arrives.

And then, he, like, very briefly — very briefly — looks in her mouth and is like, yeah, she’s got whatever — however he classified it — grade 4 or whatever he says — class 4 — and she has a lip tie, which — that had never been mentioned to us before, so it’s very much on the spot, this new piece of information.

You know, pretty quickly, the dentist diagnosed June as having a couple of ties. He confirmed that she had a tongue tie, and he said it was severe. He also said that she had tightness under her top lip, called a lip tie. And so the baby actually needed to get two cuts. And again, Lauren said that the dentist and the consultant told her how important it was for her to do this for her baby.

One of us says, like, what happens if we don’t do the procedure? , Like what are our alternatives? And it was basically like, there’s no alternative. Like, you have to do this. Otherwise, again, long —

So Lauren and her husband decided to do it. But before the procedure starts, Melanie actually stopped Lauren from coming into the room.

Melanie turned around and put a hand on my shoulder and said, oh, no. And I said, oh, am I not going with you? She goes, well, we can’t tell you no, but if you hear her cry, it’ll impact your milk supply, like, adversely.

What do I know? So I said, oh, OK. And she pulled out the white-noise machine and said, what do you want to listen to? And I had no idea what she was talking about. I had no idea what it was. And so then she just turned it on — white noise — and left.

What happens next is, Melanie turns on a white-noise machine in the room.

And that was the moment that I was like, get your baby and get out of here. And I didn’t listen to it. It was like all of my mom intuition firing, being like this isn’t right, you know. It’s like, I don’t know how to describe it, but your full body — you have to get your baby and get out of here. And I just ignored it.

She said her maternal instincts really kicked in, and she just had this instinctive fear about the procedure and whether June would be OK. But the procedure itself was very quick. Within just a couple of minutes, Melanie returns with June.

And she was screaming. Like, screaming, and so worked up. This was, like, hysterical, inconsolable. And she was also choking on something, like, gagging.

And June was so worked up. Lauren had only had her for a couple of days, but she said that this was on a different level than any other way she had ever seen June crying. And June just wouldn’t stop crying.

And she looked over to Melanie, and Lauren said that she remembered Melanie saying this was very typical. And so they pay the dentist. They pay $600 for the procedure, and then they go home.

Over the next several days, June did not get better as Melanie had assured them. You know, she was basically inconsolable, Lauren said — just crying hysterically. And Lauren and her husband — they don’t know how to comfort her. They’re new parents. They’ve only had a baby a couple of days. And they’re almost beside themselves.

There was nothing we could do. And I remember finally, I said, like, this is not normal. We’re going to an emergency room.

And they decided to go to the emergency room, where a doctor looks inside June’s mouth and finds a large sore in her mouth that he says is probably causing her so much pain.

And so he said, you know, it breaks my heart to see a sore that big in a baby this small. It was like the floodgates opened, and there was nothing but guilt and shame. Like, unmanageable guilt and shame.

Like, what have we done? Who are these people? What have I done to my baby? Will she ever be the same? Like, what did I do?

So at this point, Lauren is really understanding that her intuition about this surgery was probably right and that she and her husband may have really made a mistake with this. What does June’s recovery look like?

So June never did end up breastfeeding successfully, which was the main reason why Lauren and her husband had decided to do this procedure.

That was the whole point, right?

That was the whole point. Right. And over the next couple of years, June had a number of issues that there’s no official medical diagnosis for, but Lauren has attributed a lot of her behaviors to what had happened to her when she was just a few days old.

I mean, you couldn’t close a fridge door too loud, or else it would set her off. Or, we would attempt to take her for a stroller walk on the Greenbelt, which is the walking path, and she’d be asleep in her car seat, you know, stroller, and someone would try to pass us on their bike and ring their bell, and it would startle her, and it would just set her off. So she just was very, very, very fragile.

So Lauren just wanted to get answers, and she really wanted to hold Melanie and the dentist accountable. So she gathered all of the paperwork that she had — texts, emails, other correspondence — and she went to the Idaho Board of Dentistry, where she filed a complaint against the dentist. And then, she also went to a professional organization that certifies lactation consultants and filed a complaint with them as well.

And did she get anywhere with either of them?

At first, no. The Idaho dentistry board didn’t want to investigate, and Lauren appealed, and she lost her appeal. And she didn’t initially hear back at all from the lactation board.

No one wanted to take responsibility. That’s the thing. No one wanted to stick their neck out there. What’s the alternative? The story never gets told?

And that’s when she decided to reach out to us. And after our story came out, the lactation board finally told Lauren that they were investigating Melanie.

And Katie, you guys were reporting the story. I’m assuming you reached out to both the dentist and to Melanie. What did they say?

Beyond a very brief phone conversation that I had with Melanie in which she defended her work and she said that she had a number of very satisfied customers, she didn’t respond to detailed questions about Lauren’s story or the stories of her former clients. And Dr. Zink did not respond to our requests for comment, but he did tell the dentistry board that Lauren’s baby’s procedure was uneventful and that an extremely small percentage of patients do not respond well to the procedure.

And how big of an issue is this, Katie? I mean, how common is it for mothers to have an experience like Lauren’s?

So after we got the tip from Lauren and we dug deeper into her story, we found ourselves really surprised by how big this industry was for tongue-tie releases. And in part, it’s been driven by this movement for breastfeeding and the Breast is Best campaign and a growing number of parents who are choosing to breastfeed their children.

In turn, that has sparked this big boom in tongue-tie releases. One study that we found showed that these procedures have grown 800 percent in recent years.

Yeah. And also, as we started talking to other parents around the country, we learned that some of them had similar stories to what Lauren had told us. There’s plenty of instances where there’s no harm done to the baby at all when they get these procedures.

But we also found cases where babies were harmed, you know, where they developed oral aversions, which basically means that they don’t want to eat because they fear having anything put in their mouth, including a bottle. We found cases where babies became malnourished, had to be hospitalized. We found more than one instance in which babies had to be given a feeding tube just weeks after the procedure.

So these sounds so painful and awful for a newborn — these problems. But I guess there’s always a risk, Katie, in any medical procedure, right? I mean, how much of this is just the risk you sign up for when you agree that your baby should have a surgery?

Well, that’s true. I mean, there’s always a risk. But what you’re supposed to do is weigh the risks against what the potential benefits of a procedure are. And when we really started drilling down into what those benefits were and into the medical research, we found there just wasn’t a lot of potential benefit for these procedures, if at all, in many cases.

Really? So the procedures don’t have a medical reason to exist?

That’s right. We reviewed all of the best-quality medical research on this. And other than that old-fashioned snip under the tongue, which does show that in some cases, it can reduce pain for breastfeeding mothers, but otherwise, all of this growth and all of these other more invasive procedures — we found there just wasn’t good evidence that they helped babies. And the more we looked into tongue ties and started to connect it to the other reporting we were doing, we started to realize that it was driven by some really big forces in our health care system that really had the potential to harm patients.

So Katie, we talked about this new surge in a procedure that surgically unties infants’ tongues from the bottom of their mouths, often needlessly, sometimes even harmfully. And you said your reporting found that this surgery was actually part of a broader trend. Tell me about this trend and what’s driving it.

So that’s what this investigation was really about — to find the procedures that are doing unnecessary harm to patients and to really understand why this is happening. You know, like, what’s driving the prevalence of these procedures? And there’s just a lot of unnecessary surgeries out there, but we decided to center our reporting on three particular surgeries that had the potential to harm patients, in addition to tongue ties. We focused on a particular hernia surgery, a bariatric surgery, which can be overdone and cause harm, and a vascular surgery done on patients’ legs to help us understand the forces that were at work that were driving all of this.

And what did you find when you dug deeper into those surgeries?

Well, it’s very complex, but we ultimately found three main drivers that were underlying all of these. First, there’s a financial incentive for the doctors to perform these surgeries. There’s also a real push from the medical device companies that make these surgeries possible. And last, there’s a huge information void for solid medical advice that a lot of these doctors and companies take advantage of in order to push the surgeries.

OK, so let’s start with the money, Katie. How exactly is that incentivizing doctors to perform a lot more of these procedures? Like, what are the mechanics of that?

So the reality of our health care industry today is that in many places, even in places like non-profit hospitals, the doctors who work there are not getting a salary, a straight salary that’s just kind of, you get paid for showing up to work that day. Instead, they’re actually getting paid based on the procedures that they’re doing, how complex those procedures are, or possibly how lucrative.

And it’s not every doctor. There are still doctors that get paid salaries. But it’s increasingly the case that doctors have — at least a part of their pay is tied to the procedures that they’re doing.

Interesting. So the procedure is growing in importance in terms of actual compensation for doctors.

Right. I mean, in part, it’s kind of baked into the health care system that we’ve always had. You can even think about it as the small-town doctor who operated his own independent practice or her own independent practice. It’s essentially a small business, and they would get paid based on the patients that they saw.

But increasingly, even in, for example, large hospital systems where you might think that a doctor is just getting paid a salary to work in a hospital, in fact, a chunk of their bonus, for example, can sometimes be tied to the procedures that they’re doing, and that is increasingly the case.

Interesting.

And so one particularly egregious example of this was at a hospital that’s in New York — Bellevue Hospital. And basically, what my colleagues found there was that they had basically turned their surgery department into an assembly line for bariatric surgery, which makes your stomach smaller and can lead to weight loss. But what we found was that they were greenlighting patients that, basically, didn’t meet the qualifications for the surgery, which is a serious surgery. And what they found was that there were several situations where people had very serious outcomes as a result of getting the bariatric surgery there.

OK, so this is an extreme case of a hospital turning to a particular surgery to drive profits. And it wasn’t uncommon in your reporting, it sounds like.

No, it wasn’t the only example, but it was the most striking. And when we reached out to Bellevue, they defended their work, and they said that their practices were helping patients who wouldn’t otherwise get care. But our reporting was pretty conclusive that the program was churning through a record number of surgeries.

So what else was driving this increase in harmful surgeries that you guys found?

So we found it wasn’t just the hospitals who were benefiting. The other major player that benefits are these companies that are making the tools and the products that doctors are using during the procedures. And in order for them to sell more of their products, a lot of time, what they end up doing is promoting the procedures themselves.

So like medical device makers, like the company that made the laser in June’s surgery.

Right. And they do this in a number of ways. They’re giving them loans to help them buy the equipment, and in some cases, they’re even lending them money to help set up those clinics where the procedures are performed.

So they’re really underwriting these doctors so that they can perform more surgeries and, ultimately, sell more machines.

Yes. And the other things that they do is — the laser companies, for example — they will host webinars where they will have dentists who frequently perform these procedures show other dentists how to do the procedures. We even discovered this conference that was created by one of the laser companies, and it had kind of a wild name. The name of the conference was Tongue Ties and Tequilas.

(CHUCKLING) Right. It brought in dentists to talk about how to make money off the procedures. You know, how to promote themselves on social media, how to actually perform the procedures, and of course, when they were all done, they got to celebrate with an open tequila bar.

OK, so a lot of this really amounts to these companies trying to popularize these procedures, basically, like, to get the word out, even if the procedures don’t really work or, in some cases, cause harm.

Right. But they also play a big role in the other factor that’s driving a lot of this, which is the information that they put out there about the surgeries. These companies often sponsor research, which doctors often rely on to guide their practices. And part of what we’ve found is that it can create this echo chamber where doctors feel more comfortable and justified in doing these procedures when they have this whole alternate universe that is telling them that it’s OK to do these procedures, and in fact, it’s beneficial to patients.

So tell me about this echo-chamber effect.

The best example of this we found was a doctor in Michigan named Dr. Jihad Mustapha. He calls himself “the Leg Saver.” And what we found was that he and several other doctors do these procedures called atherectomies, which is basically like inserting a tiny roto-rooter inside an artery to get the blood flowing.

And Dr. Mustapha in particular was not only a very prolific performer of these procedures, but he actually founded his own medical conference, and he even helped start a medical journal that was devoted to using these procedures. And you know, tongue ties — there’s really no good evidence that these are actually beneficial to patients. And in fact, despite his nickname as “the Leg Saver,” one insurance company told Michigan authorities that 45 people had lost their limbs after getting treated at Dr. Mustapha’s clinic over a four-year period.

45 people lost their limbs?

I mean, that is the ultimate version of harm, right?

Right. Now, he did speak to us, and he defended his work and said that he treats very sick people. And despite his best efforts, some of these patients are already so sick that they sometimes lose their limbs.

And how much did he receive for each procedure?

Doctors like him typically receive about $13,000 for each of these atherectomy procedures.

But we found that misinformation, or poor information, also applied when doctors were learning new types of surgeries.

Really? Like how?

So one of the areas we looked at was the area of hernia surgery that I mentioned. And there’s a particular type of surgery. It’s a very complex version of a hernia surgery, called component separation. And the expert surgeons that we spoke to said that it’s difficult to learn, and you have to practice it over and over and over again to get it right. But one recent survey of hernia surgeons said that one out of the four surgeons had taught themselves how to perform that operation.

Yeah, not by learning it from an experienced surgeon but by watching videos on Facebook and YouTube.

I mean, how unusual is that? I guess, to me, it strikes me as very unusual. I mean, I think of learning about how to take my kitchen faucet apart on YouTube, but I do not think of a doctor learning about how to perform a surgery on YouTube.

Right. And it has actually become increasingly popular in recent years, and there’s not good vetting of the quality of the instruction. We even found videos on a website run by a medical device company that was intended to be a how-to for how to do these surgeries, but the video contained serious mistakes.

Wow. And Katie, all of these videos — some of them with serious mistakes — I mean, is this something that would be subject to medical regulators? Like, is there any kind of rules of the road for this stuff?

You know, there’s less than you would expect. Sometimes hospitals have rules about what sort of education their doctors need before performing a surgery. But we were surprised that there was a lot less regulation than we thought there would be and much less vetting of these videos than we anticipated.

So essentially, what you found was this complex, oftentimes interconnected, group of forces — device companies pushing their products, hospitals bolstering their bottom line, and rampant misinformation that, as you said, all really trace back to the same motivating factor, which is money. But wouldn’t the fear of being sued for medical malpractice prevent a lot of this behavior?

You know, this kept popping up during the course of our reporting. I do think we have this idea that any time a doctor does anything wrong, they’re going to get sued. But it just wasn’t always the case in our reporting. There’s a lot of statutes of limitations, time limits on when somebody can file a lawsuit, and other ways that make it somewhat hard to really hold a doctor accountable.

One example is the regulatory organizations that oversee doctors. The one doctor that I mentioned earlier — Dr. Mustapha — state investigators had found that his overuse of procedures had led people to lose their legs. And yet, he ultimately settled with the state, and he was fined $25,000. That actually adds up to about two of these atherectomy procedures.

So it sounds like malpractice is not necessarily going to be the route to rectifying a lot of this. But I guess I’m wondering if the federal government could actually rein some of this in before the patients are harmed.

It’s possible. But this is just a very difficult issue. Some of the themes that we explored in this reporting are really just firmly embedded in our health care system in the way that it works. The fact is that we have a for-profit health care system, right? So everyone, from doctors to hospitals to the device companies, benefit when more procedures are done. All of the incentives are pointing in the same direction.

And so trying to find one or two simple solutions will probably not easily fix the issue, as much as we all hope that it could.

So is the lesson here, be much more discriminating and vigilant as a patient? I mean, to get a second opinion when you’re standing in front of a doctor — or a dentist — who’s telling you that you or your baby needs a procedure?

Yes. I think that is one of the takeaways. But look, we understood that even reporting on all of this was risky, because people could hear about these harmful surgeries and start wondering if everything that their doctors tells them is a scam. And of course, while some of these procedures are harmful, a lot of procedures are lifesaving. But ultimately, for now, patients are kind of left on their own to navigate what’s a pretty complex and opaque health care system. When you have somebody standing in front of you saying, you should do this, it can be very confusing.

And this is something that Lauren talked a lot about — just how confusing all of this was for her.

There’s a lot of information that you’re getting that is truly like someone is speaking a foreign language. And because they do it all day long, it’s not user-friendly. Like, it isn’t designed for the comfort or understanding of the person receiving the information.

There is so much blind trust and faith that you have in the system, in the providers who are giving you this information. You trust, like, this is what they do all day long. So there is no real reason to question. That is the system that we have in this country.

Here’s what else you should know today. On Friday, the Russian authorities announced that opposition leader Alexei Navalny died in prison. He was 47.

Navalny, a charismatic anti-corruption activist, led the opposition to Vladimir Putin for more than a decade. His popularity was broad, extending far outside the realm of liberal Moscow. And that proved threatening to the Russian authorities, who attempted to poison him in 2020.

Navalny survived and later extracted a confession from his would-be assassin on tape. Navalny believed that Russia could be a free society, and he had the extraordinary ability, through sheer force of his personality, charisma, and confidence, to get others to believe it, too. Though he had been in prison since 2021, his death still came as a shock.

His wife, Yulia Navalnaya, made a surprise appearance at a security conference in Munich shortly after the Russian authorities announced her husband’s death.

She received an emotional standing ovation.

In Moscow, my colleague, Valerie Hopkins, spoke to Russians who were placing flowers in his honor —

— and expressing disbelief that he was gone.

Then I asked them if they believe in the beautiful Russia of the future that Navalny talked about. And they said, yes, but we don’t think we will survive to see it.

At least 400 people have been detained since his death, including a priest who had been scheduled to hold a memorial service in Saint Petersburg.

A Times investigation has found that dentists and lactation consultants around the country are pushing “tongue-tie releases” on new mothers struggling to breastfeed, generating huge profits while often harming patients.

UnitedHealth Group's Change Healthcare hit with cyberattack

Change Healthcare has been hit by a cyberattack that is causing network disruptions expected to last at least through Thursday.

The UnitedHealth Group subsidiary, which provides data analytics and revenue cycle management, first reported network disruptions early Wednesday morning before linking it to a cybersecurity issue later that afternoon.

“Once we became aware of the outside threat, in the interest of protecting our partners and patients, we took immediate action to disconnect our systems to prevent further impact,” Change Healthcare wrote on its status webpage.

The company said it expects the disruption to last “at least through the day" and it hasn't affected any other UnitedHealth Group systems, according to an update shared Thursday morning.

Pharmacies have reportedly felt the effects of network disruptions following the Change Healthcare cyberattack. Pigeon, Michigan-based Scheurer Health told the Huron Daily Tribune Wednesday the provider couldn't process prescriptions through patients’ insurance. According to the report, the health system typically sends prescription information to Change Healthcare, which sends it to payers. 

Data show the healthcare industry is a prime target for cyberattacks. Last year, more than 133 million people were affected by healthcare data breaches in the U.S., setting a record for the most affected since data was first collected in 2010.

An Optum spokesperson declined to share additional information about the attack, including its source, and instead directed Modern Healthcare to the status webpage.

In 2022, UnitedHealth Group paid cash and assumed debt to acquire Change Healthcare in a deal totaling $13 billion. The technology company was incorporated into its Optum brand. Optum touted the deal as a way to simplify clinical, administrative and payment processes.

  • Client base
  • Market niche
  • Factors for success

The executive summary lets you make a case for why your particular daycare will be successful and competitive. It is also the section that helps you sell yourself and your business to potential investors who may play a critical role in starting your daycare.

Provide a mission statement that reflects your goals for the daycare and your motivation for starting your business.

2. Management Team

Your daycare business plan should include important information about your management team. The first part of this section may address ownership of the business itself. It should outline all essential ownership details, such as:

  • Legal names of every owner

  • Management experience
  • Salary and benefits

3. Products and Services

This section lets you talk about why you are starting this business in the first place. You will outline the services or products you intend to sell to consumers. For a daycare, it should outline the following:

  • Staff costs and required credentials
  • Will you offer food, diapers, and other child-care products?
  • How will you measure sales metrics

Daycare is typically considered a service, but product sales are also often a part of this model. Your plan should adequately account for how you will handle this dual purpose and how it fits into your business model.

4. Customers and Marketing

You must identify your market and how you will get customers into your daycare. This section will focus on your area’s need for daycare services and the types of customers you expect to get.

Identification of customers is a vital part of a business plan and should include details like:

  • How many parents need childcare services?
  • What costs can families be expected to pay for your services
  • The community you will target
  • Demographic data for potential customers

With your customers in mind, you can focus on your marketing strategies. You want to differentiate yourself from competitors and ensure the community knows your new business. The daycare business plan must outline how you will achieve these goals.

A marketing strategy should combine both traditional and digital marketing methods, including:

  • Open houses
  • Word-of-mouth advertising

5. SWOT Analysis

A SWOT analysis will address your daycare’s strengths, weaknesses, opportunities, and potential threats. This analytical method examines your company and how it will operate.

It puts each of the four metrics into a visual matrix to help prospective business owners quickly identify favorable and unfavorable factors that may affect their business venture.

A SWOT analysis should focus on how your daycare services can perform against competitors. It may address factors like:

  • Any threats the daycare may face
  • Competitive advantages and why they would be successful
  • Areas that may be improved over time
  • Technology or resources the company may use to increase efficiency
  • Internal factors that may affect economic success

Detailed SWOT sections show investors you have done your homework and give you valuable data to make informed business choices.

6. Financials

A business plan should address the unique financial situation you face. Your daycare business plan should demonstrate your break-even amounts and expected profit margins.

It will also address the costs and the areas of revenue you expect. You will want to address the following:

  • Enrollment fees
  • Payment plans
  • Cost of supplies and materials
  • Employee costs
  • Regulatory costs
  • Additional program offerings and revenue
  • Necessary loans or capital improvements

A well-developed budget can demonstrate expected cash flows versus expected expenditures. Knowing how to handle these costs will be incredibly helpful in starting your daycare.

7. Operations

Your operational plan should outline specific procedures your business and staff will follow. It will list employees and those in management positions and their responsibilities.

As in your management section, a detailed “chain of command” is essential. Staff are an important part of your operations and should be detailed here.

One of the most important aspects of a daycare operation is its location. It would be best if you addressed the following:

  • The facility’s address
  • The type of area it is in (residential, commercial)
  • The space available and amenities
  • Why is it a prime location
  • Accessibility to the community for drop-off

You should also list your business hours and the services you will have available at different times. Child daycare centers differ in when they offer care—whether during the day, evenings, or even on weekends.

If you are doing other forms of daycare, you may even utilize late or overnight hours. You can decide how you run your business, but operations should be clearly outlined in your business plan.

8. Appendix

The appendix section allows you to include other documents that add to your business plan. They may include legal documents like licensing credentials, insurance information, and business formation documents.

Additional documents in this area are meant to support your business plan. The appendix might also include the following:

  • Reference letters
  • Photographs of the facility, locations, or general area
  • Market research data
  • Compliance Requirements

The appendix is a flexible section that lets you add supporting documents and other relevant information. Only add details that are helpful to your business plan. The appendix does not need to be lengthy or filled with fluff.

These borrowers are likely to be eligible for Biden's new student loan forgiveness plan

  • The Biden administration has been working on a new student loan aid package that could come as soon as this year.

While Biden first attempted to cancel student debt through an executive order, he has now turned to the rulemaking process.

  • Here's who may qualify.

Since the Biden administration's first student loan forgiveness plan was rejected at the Supreme Court , it has been working on creating a new, legally viable relief package.

That debt cancellation could come as soon as this year . The alternative plan, which has become known as  Biden's "Plan B ," could forgive the student debt for as many as 10 million people, according to  one estimate .

The U.S. Department of Education and the negotiators tasked with determining who will be eligible for the president's revised aid have identified five groups of borrowers.

1. Those who owe more than they borrowed

Borrowers with outstanding federal student loan balances that exceed what they originally borrowed may be among those who qualify for the cancellation.

A person's student debt can balloon for a number of reasons, said Nadine Chabrier , a senior policy and litigation counsel at the Center for Responsible Lending.

"Unfortunately, it is very common," Chabrier said.

Student loan servicers, the companies the Education Department contract with to service its debt, have a record of steering consumers into forbearances and deferments, she said. These options for  struggling borrowers  can keep loans on hold for many years, but interest often continues to accrue. 

Scott Buchanan, executive director of the Student Loan Servicing Alliance, a trade group for federal student loan servicers, denied that the companies benefit by veering from the government's orders.

"We are incentivized to meet the requirements that the government sets, which includes giving borrowers the benefits that the law provides," Buchanan said. "We are audited, and get business or lose it based on meeting those standards."

Advocates have also said the interest rates on federal student loans are too high, especially for borrowers from the 1980s , who have rates exceeding 8%. Current fixed rates today can be nearly as high .

2. Borrowers in repayment for 20 years or more

Those who have been carrying their student debt for decades may also benefit.

With many of the Education Department's repayment plans requiring 20 years or more of payments, such stories are common. Millions of Americans older than 60 are still paying off their student loans , research finds.

"There is both financial harm and psychological harm of being in debt for decades, especially when it feels like there is no hope that it will ever be repaid," said Persis Yu , deputy executive director at the Student Borrower Protection Center.

3. Attendees of schools of questionable quality

In its revised relief package, the Biden administration notes it is looking to include student loan borrowers who attended career-training programs "that created unreasonable debt loads or provided insufficient earnings for graduates," as well as borrowers who attended institutions with high student loan default rates.

4. People eligible for forgiveness who haven't applied

The Education Department already has several programs that lead to student loan forgiveness, and as part of its new aid package, it is looking to identify those who may be eligible but just haven't applied.

For example, the Public Service Loan Forgiveness program, signed into law by then-President George W. Bush in 2007, allows certain not-for-profit and government employees to have their federal student loans canceled after 10 years of on-time payments. In 2013, the Consumer Financial Protection Bureau estimated that one-quarter of American workers  may be eligible .

However, the technical and often confusing requirements of the plan have acted as a barrier, experts say.

Student loan servicers also earn a fee per borrower per month, which advocates say discourages transparency around loan forgiveness opportunities.

"Instead of providing borrowers with access to the affordable pathway out of debt, decades of mismanagement and abuse have left these borrowers trapped in debt like hamsters on a hamster wheel with no way out," Yu said.

5. Borrowers experiencing financial hardship

The Biden administration has also said it wants to forgive the debt of those experiencing financial hardship.

So far, it has proposed a set of factors that could identify struggling borrowers, such as those with student loan balances and required payments that are unreasonable relative to their household income, and people with high child care and health-care expenses.

It also said that financial hardship could be based on other debt obligations, disability or age, among other factors.

AT&T says cell service is back after a widespread outage and some disrupted 911 calls

AT&T says it has fully restored cellphone service to tens of thousands of customers in cities across the country whose phones lost signal overnight, causing frustration and concern about disruptions to 911 dispatches.

The FBI, in a statement Thursday afternoon, said the agency was "in contact with AT&T regarding today's network outage. Should we learn of any malicious activity we will respond accordingly."

White House national security spokesperson John Kirby also referred to the outage as he addressed reporters on Thursday. Kirby said the FBI and the Department of Homeland Security (DHS) were looking into the outage. He said the Federal Communications Commission was also in touch with AT&T.

"But the bottom line is we don't have all the answers," he said.

Around 3:30 a.m. ET Thursday, outages reported by downdetector.com suddenly spiked from just a handful, peaking at more than 73,000 by around 8:20 a.m. ET.

By late afternoon Eastern time, AT&T said it had "restored wireless service to all our affected customers."

"We sincerely apologize to them," the company said in an email to NPR. "Keeping our customers connected remains our top priority, and we are taking steps to ensure our customers do not experience this again in the future."

Kirby said the Department of Commerce faced some disruptions as a result of the outage but those were not "crippling."

Earlier, Downdetector said that Houston, Dallas, San Antonio, Chicago, Indianapolis, New York, Miami and Charlotte reported the highest number of outages. Houston had more than 2,000 reports by about 8:30 a.m. ET, while New York reported about 1,300.

However, service disruptions caused concern Thursday morning even outside those areas, with the San Francisco Fire Department announcing on X , formerly Twitter, that it was "aware of an issue impacting AT&T wireless customers from making and receiving any phone calls (including to 911)."

"We are actively engaged and monitoring this. The San Francisco 911 center is still operational. If you are an AT&T customer and cannot get through to 911, then please try calling from a landline. If that is not an option then please try to get ahold of a friend or family member who is a customer of a different carrier and ask them to call 911 on your behalf. Do not call or text 911 to simply test your phone service," the department said.

A survey conducted by the National Center for Health Statistics found in 2022 that nearly 71% of adults and 82% of children lived in wireless-only households.

Even if a cellular service provider is down, anyone with a cellphone can still place an emergency call or text, according to 911.gov . "However, calls to 911 on phones without active service do not deliver the caller's location to the 911 call center, and the call center cannot call these phones back to find out the caller's location or the nature of the emergency. If disconnected, the 911 center has no way to call back the caller," the website says.

Jared Juliano, assistant director for Prince William County Department of Public Safety Communications in Virginia, says 911 service there was never really disrupted, but calls coming in from AT&T phones did not have location information or what he describes as an "advanced caller ID."

However, he says 911 dispatchers "always verify a location even when we get locations from these numbers."

Police in the Charlotte-Mecklenburg area in North Carolina said customers were "briefly unable to contact 9-1-1. There are no disruptions to our call center's ability to receive 9-1-1 calls. Service should be returning shortly."

Other providers reported business as usual

The outage did not appear to have any real impact on other providers. Verizon said its network was "operating normally."

"Some customers experienced issues this morning when calling or texting with customers served by another carrier," a company statement said.

Likewise, T-Mobile said in a statement that its network "is operating normally."

The outages occurred around the same time as a solar flare, according to the National Oceanic and Atmospheric Administration (NOAA). The agency said the flare — "an eruption of energy from the Sun that usually lasts from minutes to hours" — reached Earth at 1:32 a.m. ET.

NOAA says such flares can affect high-frequency radio signals, but only on the sunlit side of Earth.
